On September 30, 2025 Child #1 wet themself on the playground and was escorted to the restroom by Staff #2. Once at the restroom child #1 entered and Staff #2 proceeded to go to the classroom and retrieve dry clothes. Staff#1 noticed the other staff walking away from bathroom that child was in and went to the restroom door until the child's staff returned. It was approximately30-45 seconds in break of supervision. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. Correction: A final warning was given to staff #2 on 10/1/2025 for failure to follow supervision regulation and policies. In addition, training occurred with staff #2 that day and with entire staff on October 8th during a staff meeting on active supervision regulation and policies. During renewal inspection 8-1-25, there was multiple areas of peeling paint on the building next to the children's playground accessible to the children. Pictures were taken and shown to the Executive Director. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
