Staff assigned to specific children (3270.113(a)(1))

Based on interviews with staff, the following was confirmed: On 4/7/26 at 10:35 AM, Staff Person #1, #2 and #3 were in the gym preparing to transition 16 preschool children back to the preschool room. Staff Person #1 was supervising 10 preschool children. Staff Person #2 was supervising 6 preschool children. Staff Person #3 was assisting the group. Child #1, who was assigned to Staff Person #1, was left in the gym for approximately one minute when the group transitioned until another staff person observed the child was alone. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: *Partner System for Children: Classrooms (toddlers and preschoolers) will implement a consistent partner system when children transition outside the classroom (e.g., to the gym or playground). Keeping the same partners adds an extra layer of accountability and support. *Consistent Counting Procedures: Teachers will count children at key transition points, including both the top and bottom of stairways, as well as during regular gathering times outside the classroom, to ensure all children are present. *Individual Accountability Tools: Teachers will wear their accountability system necklaces at all times when outside the classroom to support accurate tracking. *Strategic Staff Positioning: As standard practice, one teacher will lead at the front of the line, and another will remain at the back (with a third staff member positioned in the middle when applicable) to maintain supervision throughout transitions. *Clear Communication of Headcounts: Before leaving any space and upon arrival at the next, the teacher leading the line will communicate the total number of children to the teacher at the end of the line to confirm accuracy and agreement. *Use of Attendance Clipboard: Staff will carry a clipboard indicating the number of children present, providing an additional layer of accountability during transitions. Health and Safety Training (3270.31(f))

Staff person #1 and Staff person #2, who were identified as Aides, had not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): Pediatric First Aid/CPR. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: Staff #1 and Staff #2 completed Pediatric First Aid/CPR sessions on 5/20/25 (Staff #1) and 7/3/25 (Staff #2). Therefore, this was corrected upon inspection date. Inspection: Renewal. Comply with CPSL/CPSL information (3270.32(a)/3270.192(4))

The staff file for Staff person #3 was observed to contain a current National Sex Offender Registry (NSOR) check on file that was not dated prior to first day working with children (See IS CODE SHEET) as required under the Child Protective Service Law (CPSL.). Staff person #3 was also observed to have a disclosure statement dated 7/29/25 which was signed after date of hire. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: The NSOR for Staff person #3 was submitted and the disclosure statement was also signed by Staff person #3. Inspection: Renewal. Contaminated trash in closed plastic-lined receptacle (3270.67(d))

In the Preschool Room (Room 113), a trash receptacle which contained used plastic drinking cups from snack time was observed to have a drop shot lid that was open and would not retract back to the closed position. Requirement: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles. Building Surfaces (3270.76)

In the Toddler Gym, one ceiling tile was observed to have a water damage spot from a previous water leak that has since been repaired. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Toddler/preschool: updated health report every 12 months/Initial and subsequent health reports (3270.131(b)(2)/3270.182(1))

The child file for Child #1, who is a preschool age child, was observed to contain an updated health report dated 2/18/25 which was conducted more than 12 months from the previously dated 1/23/24 health report on file. Requirement: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports. Correction: The file for Child #1 now contains a current Health Assessment. Inspection: Renewal. The child file for Child #1 contained a health assessment dated 1/23/24 that was observed to lack a statement that the child is able to participate in childcare and appears to be free from communicable disease. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. Correction: The file for Child #1 now contains a current Health Assessment that has the statement stating that the child is able to participate in childcare and appears to be free from communicable disease. Health information/Initial and subsequent health reports (3270.131(a)/3270.182(1))

The child file for Child #2, contained an initial health report that is dated more than 60 days after initial enrollment. Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports. Correction: The file for Child #2 now contains a current Health Assessment. Inspection: Renewal.
