There was sharp dry glue on the bottom of the board on the left, exposed foam on a plastic pipe by the heater cover near the window, 4 exposed screws on the heater wooden cover in room #21. There was exposed foam on the column by the freezer, peeling wallpaper on the heater wooder cover accessible to children and peeling wallpaper on the freezer creating a choking hazard in room #22. There were two missing ring covers creating a hole behind the toilets in the second and last stall, The toilets in all 4 stalls were missing the anker bolts covers exposing the screws on the bottom of the toilets, and the paper towel holder was detaching from the wall in the bathroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: All identified hazards were corrected. Sharp dried glue was covered, exposed foam was covered with tape, exposed screws were secured and covered, peeling wallpaper was repaired with laminated tape, missing ring covers and toilet anchor bolt covers were replaced, and the paper towel holder was re-secured to the wall.
