Supervised at all times (3270.113(a))

On 5/20/26 it was observed that there were supervision lapses in the Early Preschool/Caterpillar classroom. Staff person 1 was observed to be caring for 7 preschool aged children. Staff person 1 entered the bathroom and was unable to 6 six of the children in their care group on several occasions. Children in care required assistance with toileting and staff was unable to see all children while assisting in the bathroom. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. Correction: Staff were retrained on maintaining constant supervision. Classroom practices no include group toileting and positioning children during bathroom times so all children can be seen, heard, assisted and directed. Inspection: Renewal. On 5/20/26, it was observed that the refrigerator in the Toddler 1/Bunny 2 classroom did not contain a thermometer. CORRECTED ON SITE. On 5/20/26, it was observed that the nap cots in the Pre K/Monkey classroom were not labeled for individual use. CORRECTED ON SITE. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. On 5/20/26, it was observed that physical surfaces were not clean and in good repair. A water-stained ceiling tile was observed in the Infant/Bunny classroom. Several water-stained ceiling tiles were observed in the Preschool 1/Butterfly classroom. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The water stained ceiling tiles in the Infant/ Bunny and Preschool 1/ Butterfly Classrooms were replaced and/or painted as appropriate. The affected areas were inspected to ensure the tiles were dry, in good repair, and free from visible hazards. On 5/20/26, it was observed that toxics were accessible to children. Sunscreen was observed in the child cubbies in the Preschool 1/Butterfly classroom. CORRECTED ON SITE. Sunscreen was observed in the bathroom in a low, unlocked drawer in the Early Preschool/Caterpillar classroom. CORRECTED ON SITE. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: All sunscreen containers that were accessible to children were immediately removed from the child accessible areas and placed in a secure location inaccessible to children. On 5/20/26, it was observed that looped blind cords were dangling at a height that allowed for a child to put their head through it, posing an entrapment hazard in the Early Preschool/Caterpillar classroom. Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The looped blind cords in the Early Preschool/Caterpillar Classroom were immediately secured and raised out of children's reach to eliminate the entrapment hazard. It was observed that the wall near the imagination station in the Preschool room (center of the room) had one area of visible peeling paint (corrected during inspection). Upon review of staff files, it was found that staff fire safety planning training were not conducted annually. The file for staff person #1 included documentation of fire safety training on 2.13.24 with an update on 3.28.25, Staff person # 5 included training on 2.15.24 and 4.16.25 , Staff person # 10 including training on 2.13.24 and 4.13.25. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.
