On 3/13/26, child 1 was left on the fenced in playground/courtyard by staff persons 1 and 2, when the class entered the building. The child was there for approximately 1-2 minutes before the staff realized they were missing. Requirement: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: CORRECTION 1: Children must be supervised at all times and staff will know the whereabouts of all children in their assigned primary care group. CORRECTION 2: The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children during transitions. The legal entity must receive DHS approval of the training content prior to scheduling the training. CORRECTION 3: The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for tracking primary care groups during transitions. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format. Correction: All staff will ensure that they have their primary care group children under supervision at all times. All staff will completed the PA Keys training: 381323 A Watchful Eye Supervision in Early Childhood. PA Key training will be completed by 4/27/26. The agreement on file for child #1 did not include the child's date of admission. (CORRECTED ON SITE) Requirement: An agreement shall specify the date of the child's admission. The daily schedule and the written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency were not posted in the school-age space. (CORRECTED ON SITE) Requirement: The written plan of daily activities shall be posted in the group space. A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children. Refrigerator (3270.107)

The downstairs refrigerator used to store children's lunches did not contain a thermometer. The bottom drawer of the refrigerator was not clean (food residue and spoiled grapes and cucumbers). Requirement: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator. Correction: The refrigerator was cleaned and a new thermometer was placed in the refrigerator. Inspection: Renewal. Inaccessible to children (3270.75(b))

Backpacks containing first aid kits were observed in areas accessible to the children on both preschool playgrounds. (CORRECTED ON SITE) Requirement: A first-aid kit must be inaccessible to children. Correction: Backpacks will be hung on the outside of the fence on the fence post. Staff have been told this is what needs to be done. Inspection: Renewal. The record for facility person #1 included just one written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person. A health assessment form was on file for staff person #2, but the medical care provider did not fill out answer to the questions. The form did not indicate if a physical was done, did not include an examination for communicable diseases and the results of that examination, and did not include the physician's or CRNP's assessment of the person's suitability to provide child care. Requirement: An adult health assessment must include an examination for communicable diseases and the results of that examination. Windows in the Teddy Bears room (2nd floor) opened more than 6 inches. (CORRECTED ON SITE) Requirement: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches. Correction: The safety bars on the windows were opened, so that the windows will not open more than six inches. Inspection: Allocated Unannounced Monitoring.
