The current health report on file for Child #4 lacked information that the child is able to participate in child care and appears to be free from contagious of communicable diseases. The report also lacked a statement that the child had received all age-appropriate screenings recommended by the American Academy of Pediatrics. Requirement: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease. A health report shall include a statement that age-appropriate screenings recommended by the American Academy of Pediatrics were conducted since the time of the previous health report required by this section. Correction: The health report for child #4 was replaced with the necessary information stating the child is able to participate in child care and appears to be free from contagious of communicable disease. Three unlabeled rest mats were observed in Classroom #5. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Clean and good repair (3270.102(a))

In Classroom 9, the PreK Classroom, one cracked bin and one cracked pencil box were observed. The cracked resulted in pinch points. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. A white vent in the outdoor play space was observed to have peeling paint around the vent. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. The file for Staff Person #1, who was identified as a Group Supervisor, lacked verification of education to qualify them for the position. The degree on file did not meet the qualifications for the human services field. Requirement: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A group supervisor shall have a bachelor's degree from an accredited college or university in early childhood education, child development, special education, elementary education or the human services field. Correction: Because the staff person #1 lacked the necessary bachelors degree to qualify as a group supervisor that staff person only qualified as an assistant group supervisor. Inspection: Renewal. The file for Staff Person #2 lacked an out of state Child Abuse Clearance for a state they established residency in within the last 60 months. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person #2 may not work in a child care position at the facility. Correction: Staff Person #2 has been removed from the schedule until they receive the complete out of state child abuse clearance. We will ensure that if a new hire has resided out of state within the last 60 months all clearances from that state they resided in will be on file prior to starting in a child care facility.
