Based on discussion with staff and video evidence, Staff #1 was observed to aggressively pick up a child, who was standing while holding onto a shelf, under their arms and place the child roughly in a sitting position on the floor. Staff #1 then picked the child up again, cradled them in their arms and proceeded to rock the child up and down using rapid, aggressive motions. Requirement: A facility person may not use any form of physical punishment, including spanking a child. Correction: Staff Member #1 was terminated. Inspection: Complaints- Legal Location. The current Pediatric First Aid/CPR Certificate in the file for Staff Person #3 was acquired after the previous certificate had expired. Requirement: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: This violation is already corrected. Staff person #1 has an updated Child Abuse certificate in their file dated on 01/23/2026. Staff #2 has an updated NSOR certificate in their file dated on 12/19/2024. Staff persons 5, 6, 7, now have updated disclosure forms in their files and staff 8 will sign the updated disclosure statement after her return to work. Observed dirt and debris under the mat of the diaper changing table in the Infant Room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Small Toys and Objects (3270.103)

Observed plastic bags, which were accessible, in an unlocked drawer in the Older Toddler Room. This presents a potential choking hazard for children who may still be placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. The mandated reporter training on file for staff person #2 was completed after their 90 days of hire on 3/12/25 (SEE LIS CODE SHEET). The mandated reporter training on file for staff person #3 was completed on 7/14/19 and 2/2/25, more than the required 60 months apart. The state police clearance on file for staff person #5 is missing the dissemination date. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: The mandated report training for staff#2, was completed on 03/12/2025, meaning it was corrected on this date. The mandated reporter training for staff #3 was completed on 02/02/2025, meaning although it was late as well, it has been completed since then. Regarding staff person #5, this person went onto their clearance portal and was able to print out the same state police clearance with the disseminated date of 06/24/24. The health and safety training on file for staff person #1 was completed after their 90 days of hire on 1/20/25 (SEE LIS CODE SHEET) Staff person #4 has acceptable pediatric first aid/CPR training in an approved curriculum; however, the trainer's name is not on the certificate therefore it cannot be assessed for whether or not they are PQAS-certified in that training topic. Requirement: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Correction: The health and safety training for staff person #1 was completed on 01/20/2025. Therefore it was already corrected, however I will explain how this will be avoided in the future in the Plan to Maintain Compliance. I have added a printed email to Staff #4's file. Our quality coach, Kathy Wolfe emailed the original CPR training that staff #4 took in 2024. The training outlines the instructor, Howard Halle. Correction: The emergency contact form and financial agreement for child #2 was reviewed and signed by the parent on 03/12/2025. Therefore it was corrected but in the plan to maintain compliance, I will explain how I will avoid this in the future. Inspection: Renewal. The file for staff person #1was missing proof of 2 years of experience to qualify them as an assistant group supervisor. Requirement: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: The staff was given a Proof of Employment form to fill out. The staff member filled out how many hours they have been working with us and that equaled the amount of hours she needed to get her to 2 years of experience and qualify as assistant group supervisor. Locked or inaccessible (3270.66(a))

A door located by the restrooms was unlocked and inside there were two large gallons of Fabuloso cleaning solution that was accessible to children. The door to the kitchen was unlocked and inside there was a floor level cleaning cabinet that was unlocked and contained bleach that was accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: I added a child lock to the kitchen and staff bathroom door. I also moved the cleaning products off the floor on the staff bathroom and put in an unreachable place. Handwashing signs (3270.82(h))

The restroom by the pre-k classroom had a small toilet that did not have a hand washing sign located above or near it. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. There were more than 12 months between the last fire detection report on 1/18/24 and the most recent which had not yet been completed . Per Chapter 3270 Act 62 regulation updates on May 24, 2021, the fire detection device or system must be tested annually by a fire safety professional. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. Correction: Our fire system was inspected on 04/03/2025. I do understand that this is over the allotted 12 months time frame for the annual inspection.
