The file for Child #1 contained an initial health assessment dated 2/2/26 and for Child #4 contained an initial health assessment dated 4/15/26, both more than 60 days beyond enrollment (SEE IS CODE SHEET). Requirement: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record must contain initial and subsequent health reports. Correction: These 2 children are siblings. The parent had recently moved on this side of town and had to find a new medical provider, which tool a little longer to get an appointment. The emergency contact form on file for Child #3 was lacking parental consent for administration of minor first-aid procedures by facility staff. Requirement: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Correction: Once informed of this error, I immediately, had the parent sign for the consent of administration of minor first-aid. Inspection: Renewal. ACIP recommended immunization record (3280.131(e))

The immunization record on file for Child #1 is dated 2/2/26 and for Child #4 is dated 4/15/26, both more than 60 days after Child #1 and Child #4's date of enrollment (SEE IS CODE SHEET). Requirement: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Correction: This violation has since been corrected. I am aware of child#1 and child #4 turning in the immunization records late. The most recent health assessment on file for Child #5 (Young Toddler) is dated 4/22/25, exceeding the 6-month update. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record must contain initial and subsequent health reports. Correction: Child #5's file lacked a health assessment within the 6 month time frame, which was an error on my behalf. The emergency contact form on file for Child #2 was lacking health insurance coverage and policy number under a family policy or Medical Assistance benefits, if applicable. The emergency contact form on file for Child #2 was lacking the enrolling parents work addresses and telephone numbers. Requirement: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Correction: The parent of child #2 was not working at the time of paperwork review. However, the parent has filled in the missing information with her new job addresses and telephone numbers. The file for Child #2 contained the original fee agreement. Requirement: A parent shall receive the original agreement. The facility shall retain a copy of the agreement. Correction: I have since made a copy of the original agreement form and I was sure to give the original form to the parent. The copy has been placed in child #2's file. Inspection: Renewal. Admission date/Application, admission, withdrawal dates (3280.123(a)(6)/3280.182(2))

The fee agreement on file for Child #3 was lacking the date of admission. Requirement: An agreement shall specify the date of the child's admission. A child's record must contain the dates of application, admission and withdrawal of the child. Correction: After being informed of this oversight, I went back in the files and filled in child #3's start date so that I would be in compliance with agreement forms specifying a child's application, admission, and withdrawal dates. Building Surfaces (3280.76)

It was observed that there was a loose board on the fence in the outdoor play space and an air conditioner with sharp corners on a table in the basement childcare space. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: I am aware of the loose board and the air conditioner with sharp corners being a hazard and that this does not comply with Code 3280.76. The sharp edges have since been taped, but will be removed away from where it is exposed to the children. A maintenance person will be making the needed repairs on the outdoor fence soon, but we will not utilize our outdoor space until the loose board is repaired. Inspection: Renewal. Comply with CPSL (3280.32(a))

The most current mandated reporter training on file for Staff Person #1 is dated 6/26/20, which is more than 60 months ago. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: The mandated reporter course will be taken and submitted as soon as possible to that I am in compliance with Code 3280.32(a), which states that operators must comply with CPSL and Chapter 3490. Age and training (3280.31(e)(4)(i))

The two most recent Pediatric First Aid/CPR trainings on file for Staff #1 are dated 2/8/24 and 3/5/26, more than 24 months apart. Requirement: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first-aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Correction: Upon notice of professional training being incomplete before the expiration, I made the necessary arrangements to have the fire safety training completed on 3/5/26. No peeling paint or plaster (3280.77(a))

It was observed that there was peeling paint on the outside kitchen and basement windowsills. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Emergency plan (3280.26(a)(6))

The documentation on file for the most recent emergency drill does not allow for documentation of the following required elements: if any special accommodations for children with special needs were required, and any observations or problems encountered during the drill. Requirement: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility. Correction: The new emergency form has now been posted and we will begin to report on problems or observations that we may encounter during our drills. Inspection: Renewal. Building Surfaces (3280.76)

The following areas were observed in the outdoor play space: small and large sticks, pieces of the fence that were loose leaving exposed screws and gaps in the fence, a tarp in the back corner of the fence that is not secured to the fence leaving a gap that children could get under, two wooden planks that are embedded into the ground which are not level to the ground leaving a gap between the plank and ground causing a tripping hazard, the outside door leading into the basement has splintered wood at the bottom, there is dirt and debris at the entrance of outside door, the screen door at the outside door leading into the basement is detached, there were empty chip bags and miscellaneous paper throughout the play space, splintered wood posts and railings on the back porch, a splintered deck board on the porch at the entrance to the outside basement door. The following areas were observed in the basement childcare space: Two ceiling tiles that were hanging from the ceiling, cracked lattice around water tank and cracked piece of paneling on the back wall causing pinch points, a portable air conditioner on the table with sharp corners. In the childcare space it was observed that the portable sink was not in working order. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: We will begin our Spring clean- up and repairs where all of these violations will be addressed. The small and large sticks will be removed, the fence will be repaired and there will be no pieces of the fence loose leaving exposed screws and gaps in the fence. The tarp in the back corner will be secured as not to leave room for children to get under. The two wooden planks will be removed, and the basement door with splinters will be repaired, and the dirt and debris will be cleaned up outside of the basement door. The screen door will be removed, and the splintered wood posts and railings on the back porch, as well as the splintered deck board on the porch at rear entrance to the basement door will all be repaired. The two ceiling tiles that were hanging down in the basement will be repaired. The cracked latticed will be fixed, as well as the cracked piece of paneling on the back wall causing a pinching hazard, and the air conditioner will be removed. Inspection: Renewal. Heat Source (3280.71)

It was observed that the stove in the kitchen that can be accessible to the children in care was lacking protective guards on the knobs. Requirement: Hot water pipes and other sources of heat exceeding 110° F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact. Correction: We will purchase safety guards and place them on the stove knobs to sure that the children are unable to turn on the stove when walking through the kitchen. Inspection: Renewal. Locked or inaccessible - in original container (3280.66(a))

It was observed that there was a can of paint and flea and tick killer on the back porch that was accessible to the children in care. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Correction: The can of paint and the can of Flea &Tick Killer was removed from the back porch and placed in an area that is not accessible to the children in care. Inspection: Renewal. The two most recent mandated reporter training certificates on file for Staff Person #2 are dated 6/11/18 and 1/25/24 which are more than 60 months apart. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: We are aware that staff #2 receive her mandated reporter training certificate more than 60 month apart, but it was corrected when staff became aware of the error. Inspection: Renewal. The file for Staff Person #1 contains NSOR certificates dated 2/12/20 and 3/12/25, the file for Staff Person #2 contains NSOR certificates dated 3/11/20 and 3/19/25, which are both more than 60 months apart. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL Correction: Both of our NSOR certificates had expired and had been corrected prior to inspection. The file for Staff Person #2 includes 8 of 12 required annual training hours completed between 12/19/23-12/19/24. Requirement: A staff person shall obtain an annual 12 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility. Correction: We are aware that staff #2 completed 8 of the required 12 training hours. To correct this, we borrowed 4 training hours that she has taken so far in this year of 2025. Letter to parents (3280.26(e))

The facility lacked a letter explaining the emergency procedures that is to be given to each enrolling parent. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. General Health and Safety (3280.20)

It was observed in the outdoor play space that there was a bucket of standing water with insects in it. In the childcare space it was observed that there was a pair of adult scissors on top of the cubbies that were accessible to the preschool child that was in care. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The bucket was emptied and removed from the premises the afternoon after inspection, and the adult scissors were also put away and placed in the teacher's bin out of reach from the children. Clean and good repair (3280.102(a))

The following were observed in the outdoor play space: cracked green bin on the porch, a toy kitchen door by the garbage can with sharp edges, a cracked yellow milk crate, all riding toys, equipment and toys were visible dirty and not in good repair, Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The cracked green bin, the toy kitchen door with sharp edges, and the cracked yellow milk crate were all removed that afternoon. The riding toys and remaining equipment will be cleaned when we begin our Spring clean-up. These toys and equipment are not in use during the winter and spring seasons. Inspection: Renewal. Infant sleep position (3280.119)

It was observed that an infant was sleeping in their car seat which isn't a recommended sleep position by the American Academy of Pediatrics. Requirement: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. The parent receives the original agreement. Correction: On 5/19/25, I made copies of the original agreement and placed the copy in the children's files.
