Small Toys and Objects (3270.103)

Peeling tape was observed on the bottom of a door near the bathroom, and accessible to children who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: The tape identified on the bottom of the door near the bathroom was removed and the area was repaired at the time of the inspection. No peeling paint or plaster (3270.77(a))

On the outside of the entry door, red peeling paint was observed. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: A contractor has been notified to evaluate the condition of the front entry door and complete the necessary repairs. The peeling paint will be removed and the door will be repainted. In the middle space of the Main Room, peeling paint was observed on the side wall. In the bathroom (right), peeling paint was observed behind the toilet, along the baseboards, and along the back of the sink. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Peeling paint on he side wall in the Main Room was corrected the day of inspection. Contractor is expected to come in the next few weeks to fix peeling paint in the right bathroom behind the toilet, along the baseboards ad the back of the sink. Inspection: Renewal. Building Surfaces (3270.76)

In the Gross Motor Room, the carpet was observed to be peeling and not in good repair. In the bathroom (left), the linoleum floor was peeling in the doorway, creating a tripping hazard. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The carpet in the Gross Motor Room and the linoleum floor in the bathroom was corrected the day of inspection with duct tape. Inspection: Renewal. The emergency contact form for Child #2 lacked the release person's phone number. Requirement: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. Correction: I contacted the mother of Child #2 on the day of the inspection and obtained the emergency contact person's phone number, which has since been added to the child's emergency contact form. The file for Child #1 contained an emergency contact form that had updates more than 6 months apart. Requirement: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Correction: I contacted the mother of Child #2 on the day of the inspection and obtained the emergency contact person's phone number, which has since been added to the child's emergency contact form. The most recent health report form on file for Child #1 (a young toddler) is dated 8/9/24, more than 6 months prior. Requirement: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. A child's record shall contain initial and subsequent health reports. Correction: I requested an updated Child Health Report from Child #1's legal guardian, at which time I was informed that the child's last day of care will be June 19, 2025, due to the family's upcoming move.
