A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. In Room 3, two bottles of raid and a bottle of Ammonia, labeled keep out of reach children were in unlocked closet accessible to the children in care. In Room 3, there was a Swiffer with cleaner labeled keep out of reach of children in the classroom and accessible to the children in care. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: A lock was placed on the closet and the bottles were also moved to a higher location. The swifer mop with cleaner was also moved to the now locked storage closet. Inspection: Renewal. Building Surfaces (3270.76)

The Outdoor play space had a metal spike that was sticking up creating a protrusion hazard for the children in care. In Room 5 there were two ceiling tiles that had water marks. The frame of the door in Room 5 was cut leaving exposed sharp metal creating a cutting hazard to the children in care. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Foam cushion was placed and wrapped around the metal spike tightly. The frame of the door in room 5 was also filled in with a soft fabric to cover the potential sharp edges.
