Toddler and preschool children (3270.55(a))

On 5/28/26 at approximately 1:15PM during naptime, it was observed that Staff Person #1 was caring for 3 Older Toddlers and 13 Preschoolers who were napping and 3 other Preschooler who were up and sitting at a table in the preschool room. At approximately 1:20PM during naptime, it was observed that Staff Person #2 was caring for 1 Infant, 7 Young Toddlers, and 1 Older Toddler in the Infant/Toddler Room. Requirement: The staff:child ratio while toddlers and preschoolers are napping is one staff person to 10 young toddlers, one staff person to 12 older toddlers, one staff person to 20 preschool children. TIERED LIS: 1. Ratios must be maintained at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of in person training conducted by the ELRC regarding maintaining ratios of children. The operator shall provide a date for when this training will be completed. Correction: On 5/28/2026, the facility was immediately returned to compliance by ensuring adequate staff coverage was provided to maintain required staff-to-child ratios. The director reviewed staff assignments and supervision with staff. The facility ha registered staff for ELRC ratio training and will ensure all staff complete mandated training regarding maintaining ratio at all times. 1. Staff were out of ratio on 5/28/2026 during naptime, ratios will be maintained throughout the facility at all times beginning on 5/29/2026. 2. Other staff on premises (3270.55(b))

On 5/28/26 at naptime from approximately 1:30PM to 2:05PM it was observed that there were 2 staff members at the facility caring for 1 infant, 7 Young Toddlers, 4 Older Toddlers, and 16 Preschoolers, which are not enough staff at the facility to maintain ratios if naptime ended or an emergency occurred. Requirement: Staff persons who are on duty but are not providing child care during naptime shall remain in the child care portion of the facility premises. Correction: Upon notification of the violation, the facility immediately reviewed nap time staffing procedures. On 5/29/26 Staff were instructed that when staffing levels do not permit, employees on duty who are not actively providing childcare must remain in the childcare portion of the facility premises during nap time. In the Preschool Room the nap mats were observed with less than two feet of space on three sides while in use. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Upon notification of the violation, the nap mats were immediately repositioned to ensure a minimum of two feet of space on all sides of the mat while in use. Classroom furniture was rearranged to provide adequate space for proper mat placement. Staff were reminded of the spacing requirements for rest equipment and proper classroom setup during nap time. Requirement: An operator shall notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the child care facility. Diapering area not used for food prep (3270.135(c))

It was observed in the infant/toddler room that staff were using the same sick to wash bottles and dishes as well as using the sick for handwashing after diapering. Requirement: The diapering area may not be used for food preparation or food service. Correction: The facility immediately changed procedures so that bottles and dishes will be washed only in the main classroom/kitchen sink. During the renewal inspection, 5 preschool age children were observed in the preschool room, and 4 toddler age children were observed in the toddler room without their emergency contact information in the space where they were receiving care. Requirement: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space. Correction: The children's emergency contact information was immediately placed into the designated emergency box(es) to ensure required information is present and accessible in each child care space while children are in care. Stairs, exits, etc. unobstructed (3270.91(a))

It was observed that there was a crib that was blocking the entrance to the toddler room, a pack n play blocking the entrance to the play space, an easel, Christmas tree and other items that were being stored in the hallway of the preschool/school age room that leads to an exit. The front exit door by the preschool/school age room was locked by 3 key locks and the keys were unavailable to open the door quickly in case of an emergency, and the red door in the school age room was locked from the outside making it unable to exit from the childcare space to the nearest exit. Requirement: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Correction: The hallway and storage areas were decluttered and items blocking this hallway were removed or relocated to maintain a clear walkway and unobstructed path of egress. The infant room removed cribs and playpens from doorway areas to maintain clear access at all times. The red door lock was changed so that door remains unlocked/easily accessible at all times, and the exit door hardware and handle were replaced to allow easier exit in the event of an emergency without requiring a key. The first aid kit in the preschool room and in the school age room was lacking scissors. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. Correction: Scissors were added to the first aid kits in the preschool/school age room to ensure all required first aid supplies are present and complete. Inspection: Renewal. A first aid was observed on the table in the preschool room making it accessible to the children in care. Requirement: A first-aid kit must be inaccessible to children. The water temperature in the bathroom located in the preschool/school age room was measured at 113° F. Requirement: Hot water temperature, in areas accessible to children, may not exceed 110° F. Correction: The hot water source in the preschool/school age bathroom was addressed immediately. The hot water was turned off and will not be used until the water temperature is adjusted to meet the regulatory requirements and remain at or below the required temperature. Inspection: Renewal. It was observed that a bottle of hand sanitizer was beside food items on the table in the kitchen area. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. Correction: The bottle of hand sanitizer was immediately removed from the kitchen area and placed in a locked closet away from food items and food preparation areas. It was observed that there was a bottle of foaming Clorox cleaner that was under the sink in the unlocked staff bathroom. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: A lock was installed on the cabinet under the sink, and the staff bathroom door was secured to ensure cleaning products remain inaccessible to children. The two most recent State Police Clearances on file for Staff Person #1 are dated 7/9/20 and 7/10/25, which are more than 60 months apart. Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Correction: The staff member's State Police Clearance has already been updated and is currently on file. The facility reviewed personnel records and identified that the renewal timeframe exceeded the required timeframe. It was observed in the preschool/school age room that there was an extension cord that was going across the floor from the outlet to the timecard that could cause a tripping hazard. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The extension cord creating a tripping hazard was removed immediately and the timecard was relocated to eliminate the need for a cord across the floor. Staff assigned to specific children (3270.113(a)(1))

Per the facility's self-report and staff statements, on 2/6/26 a toddler aged child (Child #1) had eloped from the classroom while Staff Person #1 was assisting another child for dismissal. Staff #2 alerted Staff #1 that Child #1, who was assigned to Staff #1's group, was in the entryway hallway unattended, which the parent of Child #1 witnessed upon arrival. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff #1 received retraining on active supervision and responsibility for assigned children, including maintaining physical presence with children and knowing the names and whereabouts of all children in their assigned group at all times. All staff reviewed supervision procedures to ensure each staff member is assigned specific children and remains physically present with their group at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. (The operator shall provide a date for when this training will be completed.) Correction: 3/12/2026- Guard gates were placed between each classroom entry/exit to prevent children from self transportation without the help of an adult to accompany them. 2/6/2026- Toddler and Infant room has become fully widened to have a full clear view of the room and for better supervision of all children in the room. Tiered 1. 2/6/2026- Children will be supervised at all times. 2. 4/10/2026- All staff will have completed a minimum of 2 hours of supervision training. Inspection: Complaints- Legal Location. The 30-Day fire detection log on file at the facility listed dates of testing of 5/23/25 and 6/24/25, which is more than 30 days between tests. Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #4 may not work in a child care position at the facility. Mixed Age Level (3270.52)

On 12/11/25 at approximately 10:30AM, Staff Person #1 answered the door leaving 11 preschool aged children and 1 older toddler with Staff #2 in Room A. While Staff #1 was at the door, Staff #2 exited Room A leaving 11 preschool aged children and 1 older toddler alone without a staff member present. At 10:33 AM, Staff #1 named and identified their supervision group of 5 preschoolers and 1 older toddler. At 10:34 AM, Staff #2 returned to Room A and identified 6 preschoolers in their supervision group. At approximately 10:45AM in Room B, Staff Person #3 and Staff Person #4 were observed in ratio and caring for children. Staff #3 and Staff #4 both named and identified the 4 young toddlers and 2 Older toddlers in the room. Staff #5 entered Room B at approximately 10:47 AM to work with Staff #3 and the 4 young toddlers and 2 older toddlers. At approximately 10:48 AM, Staff #5 stated the facility did not have a file for Staff #4. Staff #4 was not permitted to be caring for children and left the childcare space. Requirement: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level).
