The file for Staff Person #2 contained a written evaluation dated 3/2/26 but was not within 12 months of hire. The file for Staff Person #3 contained written evaluations dated more than 12 months apart (3/31/25 and 4/9/26). The file for Staff Person #4 did not contain a written evaluation within the last 12 months. The trash receptacles in the girls' and boys' restrooms were not equipped with lids. A handwashing sign was not posted near the toilet in the children's restrooms in Room 104 and in the Toddler Room. In the girls' restroom next to the gym, a handwashing sign was not posted near the toilet in the second stall. Requirement: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility. In Room 305, two first aid bags were observed placed on chairs and were accessible to children in care. Requirement: A first-aid kit must be inaccessible to children. Clean and good repair (3270.102(a))

In the AP-1 room's closet, a children's foam climber block was observed to be torn on the corners, exposing the interior and was not in good repair. Plastic bins containing children's toys were observed to be cracked with rough edges and not in good repair in Room 107 (2 bins), Room 102 (1 bin), and Room 105 (1 bin). Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. SECOND CITATION The file for Facility Person #3 did not contain documentation of fire safety training within the last 12 months. Previously cited on SIN-00263788. The files for all other Facility Persons are now in compliance. Requirement: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.
