There was a snack cheese tray in a child's cubby in the hallway. Requirement: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated. Disposable diapers (3270.135(a)(3))

Staff # 2 kept a soiled diaper on the changing table until she was done with the diaper change, instead of discarding it immediately into a plastic-lined, hands-free covered can. Requirement: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Correction: Staff #2 corrected, then changed and infant baby and discarded diaper immediately into a plastic-line hands free covered can. Inspection: Renewal. Child's hands washed (3270.134(a))

Staff # 2 did not wash the child's hands after diapering. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. There were 4 mats with less than 2 feet of space required on three sides of a mats while in use in the transition room. There were 2 mats with less than 2 feet of space required on three sides of a mats while in use in the preschool 2 room. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Staff repositioned the 4 rest mats that had less than 2 ft. Of space required on three sides, in the transition classroom. Staff also repositioned 2 rest mats that were less than 2ft. space required on three sides, in the preschool classroom. Clean and good repair (3270.102(a))

There was a big clear bin with broken sharp edges in the Infant 3 room. There was a cracked clear bin in the infant 4 room. There was a broken pink plastic bin and broken clear bin in the SA 2 room. There was a cracked clear bin, and the water table had a broken corner creating sharp edges in the preschool 2 room. The following rooms had wooden unstable cubbies with lose exposed screws .2nd floor toddler room with pink walls, 2nd floor toddlers' room with green walls and the 1st floor hallway by the infant room. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: Staff removed and replaced cracked clear bins in the infant 3, infant 4 and preschool rooms. Staff also removed pink bin in young school age classroom. Staff covered broken corner on side water table in preschool 2 classroom with electrical tape preventing any sharp edges. Operator tightened loose screws in cubbies located on the second floor in toddler classroom, and on the first-floor hallway near infant 1 classroom. No peeling paint or plaster (3270.77(a))

There were two peeling paint spots on the 2nd floor hallway wall. There was peeling paint on the purple wall by the door frame in the transition room. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: Staff covered pant spots on second floor hallway wall with tape, until operator has wall completely repainted. The wall on second floor is now free of any further peeling paint or damage surface. Staff also covered peeling paint by the door frame in transition classroom with tape. Toxic plants not permitted (3270.66(d))

There was a Wax-begonia plant consider toxic in the preschool 2 classroom accessible to children. There were Reynoutria Sachalinensis plant consider toxic in the play yard area accessible to children. Requirement: Toxic plants are not permitted in a child care space. Correction: The plan was removed from the classroom. Operator also cut down plants considered toxic in play yard are, also placed deer fencing around the area to block/ cover are to keep children, from coming in contact with any plants. There was a bottle of Freshsecent Instant Hand Sanitizer label " Keep out of reach of children" on top of the entry way table accessible to children. Requirement: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. Correction: Operator removed hand sanitizer bottle from on top of window ledge located above table in entry way to cabinet located in the office out of reach of children. Inspection: Renewal. Staff person #1 has not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated (6/24/20). Requirement: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Correction: Operator has staff #2 completed mandated reporter training, staff #2 now has updated mandated reporter training on file. Inspection: Renewal. Staff #1's file contained an FBI clearance that was unclear making it impossible to verify the date on the clearance. A clear copy of the clearance was found and submitted on 5/20/25. In the outdoor space within the grass area just past the tires was poison ivy which is toxic to children. Requirement: Toxic plants are not permitted in a child care space. In the PS 1 room the right window was open without a screen. Requirement: Windows or doors used for ventilation shall be screened when open. Correction: The operator had window without screen shut and locked. Also put a note on window advising staff to keep window without screen closed. Inspection: Renewal. Building Surfaces (3270.76)

The press bar on the hallway door going upstairs was missing the end cap exposing screws and making it a pinching hazard. The bracket at the top of the first handrail on the left-hand side was broken causing it to be loose and unstable when grasped. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The operator covered the pressbar on the upstairs hallway door with missing end cap with electrical tape therefore it no longer exposes any screws or potentially making it a pinching hazard. In the PS/SA room upstairs there was peeling paint under the middle window. In the outside area along the building wall was peeling paint. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: The operator covered peeling paint in the PS/SA room under the middle window with tape and covered peeling paint on the outside building with exterior paint/putty material. In the PS 1 room there were small Linkin logs and small pieces of crayons that had a diameter of less than 1 inch accessible to children who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. The letter provided to the parent of each enrolled parent explaining the emergency procedures did not contain accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions. Requirement: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan. Correction: In the parent letter provided to the parent of each enrolled child, the operator included accommodations explaining emergency procedures for infant, toddlers, children with disabilities, and children with chronic medical conditions. In both upstairs toddler rooms are tall cabinets that are not secured and unstable when touched. In the upstairs PS/SA room there were 2 tall red and white shelves that were not secured and unstable when touched. The cubbies in the upstairs and downstairs hall were not secured and unstable when touched. In the infant 1 room there was a tall shelf between two cribs that was not secured and unstable when touched. Requirement: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs. Correction: The operator nailed tall cabinets in both upstairs toddler rooms, 2 tall red and white shelves in PS/SA room, and in the infant room to the walls in each room, making them stable and secure Inspection: Renewal.
