Children have never had access to the space behind the washer and dryer, and the soot presents no direct contact risk during routine handwashing. However, to ensure full compliance and eliminate any future concerns: As previously noted, our program will no longer use the laundry room for handwashing. A new sink is being installed inside the restroom for both children and staff to use for proper hand sanitation, removing the need for any child access to the laundry area. Inspection: Renewal. Observed in the following areas: Loose hanging wires from the ceiling in the laundry room (handwashing area). Holes in the wall between the bathroom and laundry room. Preschool Room: holes in the wall in the corner by library, loose ledge where sleeping mats are stored, which creates a potential toppling hazard, two ceiling tiles separating in the dramatic kitchen play area. Dining Room/Living Room: peeling tape and screws loose on the swivel door between the two rooms, which creates a potential pinch hazard. Front outdoor play space: light fixture above door detached, drain pipe broken and cover not on, which creates a sharp edge. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Laundry Room (Former Handwashing Area): Loose Hanging Wires from Ceiling: These wires are not live and remain from a recent electrical upgrade. They are located high on the ceiling and are completely inaccessible to children¿an adult would require a step stool to reach them. Plan of Correction: The wires will be properly capped and secured to ensure safety and visual compliance. Holes in Wall Between Bathroom and Laundry Room: These will be patched and repaired to maintain a safe and sanitary environment. Change in Usage: This area will no longer be used for children¿s handwashing. A new sink has been installed inside the restroom to meet sanitation and handwashing requirements. Children will no longer access the laundry room. Preschool Room: Holes in the Wall (Corner by Library Area): These holes will be sealed and repainted to restore the area and eliminate any concern. Loose Ledge Where Sleeping Mats Are Stored: This area presents a potential toppling hazard. Plan of Correction: The ledge will be repaired and securely fastened. Ceiling Tiles Separating in Dramatic Play Area: The separation occurred due to a small cut made by a contractor during outlet installation. While it does not pose a hazard, Plan of Correction: We will have the ceiling tiles re-secured or replaced to restore the ceiling¿s integrity and ensure a safe environment. Infant/Toddler Room (Near Dining Room/Living Area): Swivel Door Between Rooms ¿ Peeling Tape & Loose Screws: These conditions may create a pinch hazard. Plan of Correction: The swivel door will be fully repaired, with tape removed and all hardware secured to eliminate risk. Upon noticing this, the Preschool Teacher immediately removed the items and placed them in an area that was out of the children's reach. Additionally, during the observation of the toddler classroom, it was noted that two Pack 'n Plays had loose-fitting sheets. Correction: Effective immediately, we have adjusted our mealtime procedures to ensure that no more than twice the measured capacity of the space is exceeded at any time. We are strictly adhering to this requirement to maintain compliance and ensure children's safety and comfort. As follows, when the capacity of a space is exceeded while children are eating, the number of children present in the space we will not have more than twice the measured capacity of the space. The back outdoor play area does not have a means of restricting access to the upper area where garbage is collected. Observed two ladders and a cinder block in this area, which create potential toppling hazards. The gate in the front outdoor play area, which leads to a busy street, is damaged and does not restrict access to the street. The lower connecting pole to the gate is missing and the latch does not lock securely. Requirement: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions. Correction: The area observed during the inspection is not part of our designated childcare outdoor play area. I explained this to both inspectors present at the time of the visit. Therefore, I am unsure why this area was included in the Plan of Correction. However, in the interest of compliance and continued safety, I will follow the instructions as advised. It was noted that the upper area, where garbage is collected, does not currently have a means of restricting access. The two ladders observed in this area are used for maintenance purposes only and are not accessible to children. The cinder block that was seen in this area will be removed immediately to eliminate any potential hazard. Regarding the gate in the front outdoor play area that leads to a busy street: it was observed that the gate is damaged, and the lower connecting pole is missing. These issues are all related to the same gate structure. Although the correction appears to be worded in multiple parts, I understand the concern and will ensure that the entire gate, including the pole, latch, and locking mechanism¿is either repaired or replaced. Given its location, it is not accessible to children and does not pose a direct risk. That said, our program is committed to maintaining a safe environment at all times. We ensure that all accessible outlets throughout the facility are properly covered with child-safety devices. Although this specific outlet is completely out of reach for children, if DHS recommends that the cover be replaced regardless of accessibility, we will follow through and make the correction as advised. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Correction: Yes, the following items were observed in areas that were not accessible to children: Laundry Room: An empty can of paint and disinfecting wipes were stored on a high shelf that required the use of a step stool to access. This space was being used by our Preschool class for handwashing. To maintain compliance and avoid any future concerns, we will no longer use the laundry room for handwashing. A new bathroom sink has been added for the Preschool class, and children will no longer have access to the laundry area. Kitchen Area: There is no cleaning supplies stored next to the kitchen sink, only cans of paint nearby. However, to ensure compliance and safety, all cans of paint will be removed from the kitchen area immediately. Back Outdoor Play Space: The unlocked shed will be secured with a lock as soon as possible to restrict child access.
