On the grassy play space, there were multiple metal wires on the chain-link fence that were protruding out with sharp edges and were a visible hazard. A facility person's record shall include verification of child care experience, education and training prior to service at the facility. Correction: For staff person #1, a letter with verification of the staff person being enrolled in an approved curriculum by the Department of Education and completion of the required classroom training will be obtained to qualify the staff person as an aide. Correction: A Building Blocks Child Center employee will be present at all times when an outside professional is working with a Building Blocks child, unless the center has obtained written parent permission for a professional from an outside agency to work alone with their child while a Building Block's staff member is not present. The director has obtained parent permission for children receiving outside services to work alone, unsupervised by Building Block's staff, with their child. Correction: The maintenance manager has ordered playground surface repair kits from "Trassig - The Playground People" to fix the patches on both play areas needing repair. In addition to this, the center has one 5 gallon bucket of binding agent (from last year's use) and has purchased a second 5 gallon bucket of binding agent to then give each playground surface (preschool and toddler) a top coat after the repair patches have been applied. In the pre-k counts room, handwashing signs were not posted at the two toilets in the stalls in the girls' bathroom and the one toilet in the stall in the boys' bathroom. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
