In the pre-k/preschool room, the handle on the toy fridge door was loose with the screw at the top of the handle missing resulting in the handle not being secure. In the pre-k/preschool room, there were two rest mats that had tears in the fabric exposing the foam inside. On the outdoor preschool play space, the frog bouncer embedded equipment nearest the outside fence had a crack that was a few inches long on the right side of the equipment. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: In the pre-k/preschool room, the loose handle was removed from the toy fridge. The two rest mats that had tears were removed from the room and discarded and replaced with new rest mats. The crack on the frog bouncer on the outside play space was filled with caulking according to the directions on the caulking. Inspection: Allocated Unannounced Monitoring.
