On the outdoor preschool play space, there were multiple steps/platforms on the embedded play equipment that had tears in the surface and had metal support bars that were starting to protrude through the steps/platforms and were a visible hazard. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: The preschool play space will remain closed until a permanent fix can be made. Until the play structure is fixed, the other play space will be used for all ages. Inspection: Renewal. In the infant room with an infant and multiple 1 year olds that were walking and able to wash their hands, the sink next to the changing table lacked a platform or steps and was unable to be reached by the children after diapering to wash their hands. Requirement: Toilets and sinks shall be at proper heights for children using them or shall be easily approached by means of platforms or steps. Correction: A platform step stool was purchased and placed next to the changing table for children to use to wash their hands. Inspection: Renewal. Clean and good repair (3270.102(a))

In the pre-k/preschool room, the handle on the toy fridge door was loose with the screw at the top of the handle missing resulting in the handle not being secure. In the pre-k/preschool room, there were two rest mats that had tears in the fabric exposing the foam inside. On the outdoor preschool play space, the frog bouncer embedded equipment nearest the outside fence had a crack that was a few inches long on the right side of the equipment. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Correction: In the pre-k/preschool room, the loose handle was removed from the toy fridge. The two rest mats that had tears were removed from the room and discarded and replaced with new rest mats. The crack on the frog bouncer on the outside play space was filled with caulking according to the directions on the caulking. Inspection: Allocated Unannounced Monitoring.
