Staff assigned to specific children (3270.113(a)(1))
At approximately 10:30am, two staff and fourteen preschool age children were observed. During the onset of supervision and ratio, when staff were asked to identify and name the children whom they were responsible for, Staff person #2 identified and named seven preschool aged children and Staff person #3 identified and named six preschool aged children thus leaving one child unaccounted for. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: Staff met and discussed the child that was overlooked within the overall count. Child arrived later and staff failed to add tag to their lanyard. Tag was added once it was discovered. Director will meet with staff, providing training and resources regarding supervision, ratio, etc. that maintain the safety and well-being of our classrooms and center. Inspection: Renewal. The staff file for Staff person #1 was observed to contain fire safety trainings that were conducted more than 12 months apart; those fire safety trainings are dated 3/31/25 and 4/6/26. Requirement: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. Staff person #1 and Staff person #2 have not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): Pediatric First Aid and CPR. Requirement: Staff person(s) shall complete professional development in the topics of 3270.31(f)(1 -- 10) within 90 days of hire. Staff person #1 and staff person #2 will have until 11/13/25 to complete the required training. Until such time as the required training has been completed, staff person #1 and staff person #2 must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation. If there are no staff available to supervise staff person #1 and staff person #2, staff person #1 and staff person #2 may not work in a child-care position at the facility. Correction: Staff will continue to be supervised by another staff person who has all preservice trainings completed. The immunization record of Child #1 was observed to be missing the following vaccines: 4th dose of DTaP vaccine, 2nd and 3rd dose of Hib vaccine, and 2nd, 3rd, and 4th dose of Pneumococcal vaccine as recommended by the ACIP and did not have an acceptable exemption on file. Requirement: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings). Child #1 must be dismissed from care by close of business 4/1/25 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter. Correction: Child #1 was updated the day of licensing renewal, at the end of the day. Guardian provided director with a hand-written exemption letter, stating that due to their strong person beliefs Child #1 would no longer be receiving further vaccinations. Guardian wasn't unable to provide said health report, so the child was unenrolled as of today. An email had been sent to inspector, showing what the guardian provided initially. Credentials did not seem to match, the date had not been changed, so without hearing back the decision was made to unenroll Child #1 until an adequate health report can be provided. At approximately 1:00pm in the Toddler Area, during nap time, a toddler was observed sleeping on a cot that was placed next to a shelving unit not allowing for 2ft of space on three sides of the cot while in use. Requirement: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use. Correction: Staff and director will ensure that sleeping mats/cots are placed within regulatory requirements when in use. Sleeping mats/cots will allow for at least 2 feet of space on three sides of the mat/cot to allow staff access to the children. Inspection: Renewal. Correction: The fastener on the felt board was removed completely (even from back of board), to ensure that no small parts were in the Toddler classroom. Correction was made during licensing renewal. Building Surfaces (3270.76)
In the Preschool Room 2 the bottom corner of a grey cove base located to the left of the wooden block area was observed to be detached from the wall causing it not to be in good repair. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: Grey base trim was removed and reapplied by landlord's maintenance team. Pictures will be sent of this repair to inspector. However, the repair job was not "adequate" in director's opinion, so a layer of duct tape was applied to ensure that grey trim would stay flush again the wall/floor, not peeling away further. Pictures of this additional repair will also be emailed to inspector. *As of 04/11/25, building maintenance showed up to completely remove and repair trim. In the Preschool Room 2 a stapler and stapler remover were observed stored inside an unlocked drawer of a black/translucent desktop storage organizer which was on top of a shelf accessible to the preschool children in care. Requirement: Conditions at the facility may not pose a threat to the health or safety of the children. Correction: The caddy containing a stapler and staple remover was immediately placed in a locked cabinet within the classroom. Access to that caddy is now accessible to staff only, so as not to pose a threat to the health and safety of the children. Correction was made during licensing renewal process. Inspection: Renewal.
