In the Young Toddler Room one Pack n Play Playard was observed to lack the child's name. In the 3's and 4's Room, four mats were observed to lack the names the children who use them. Requirement: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Correction: Pack n Play was labeled, and rest mats were assigned and labeled. At approximately 3:50pm a young school age child was brought into the facility by Staff person #1 and given a snack and child began consuming the snack without having their hands washed. Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: Child was directed to wash their hands once it was recognized they hadn't. Provider will ensure child's hands are washed before meals, snacks, after toileting and after being diapered.
