In the hallway between Room #1 (Infant) and Room #3 (Older Toddler), one ceiling tile was observed to not be kept clean as evidence by having a water spot from a previous leak. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The center supervisor contacted building maintenance to inspect the ceiling tile. Upon review of the facility's 30-day fire testing documentation, the following tests were observed to have been conducted more than 30 days apart: 10/17/25 - 11/17/25 (31 days) and 3/2/26 - 4/3/26 (32 days). Requirement: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met. In the main hallway, outside of the Preschool Room, 3 ceiling tiles were observed to not be kept clean as evidence by having water spots from a previous leak from the roof that has since been repaired. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The issue was repaired on the exterior and interior; maintenance did not replace the interior tiles immediately when they were repaired.
