Staff person #1 and #2 have documentation of diplomas from the Dominican Republic that have not been translated or evaluated for equivalency. Requirement: A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity. Correction: Both diplomas are now being up through the process of being evaluated for equivalency through the pa keys website. Inspection: Renewal. Adult Hygiene (3280.152)

During the time of inspection, it was observed that facility person #1 did not wash their hands after diapering a child. (Corrected on site) Requirement: A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child. Correction: Upon being told this the staff member washed their hands and will continue to do so after diapering a child. Inspection: Renewal. During the time of inspection, it was observed that the child's hands were not washed after diapering. (Corrected on site) Requirement: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. Correction: Upon being informed this the child's hands were washed and going forward after being diaper the child's hand will be washed. Inspection: Renewal. The facility's general daily schedule was not included in the parent handbook. (Corrected on site) Requirement: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements. Bottles labeled (3280.166(4))

The bottle for Child #1 was not labeled with the child's name. Requirement: Disposable nursers and bottles shall be labeled with the child's name. Has all items/Transportation First Aid Kit (3280.75(c)/3280.178)

The first aid kit located in the van used to transport children, did not contain soap or scissors. Requirement: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves. A first-aid kit, including the contents as specified in §3280.75 (relating to first-aid kit) shall be in the vehicle when children are being transported.
