In the back-room area, hand sanitizer labeled keep out of the reach of children was observed on a shelf and accessible to children. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. In the hallway near the back door a broken drywall hole was observed to contain broken tape. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: To correct the broken dry wall, I replaced the tape to cover the patch that is visible. Inspection: Appeals - Unannounced Monitoring. Small Toys and Objects (3280.103)

At the gate near the entrance, the metal gate loop (less than 1 inch in diameter) was observed to be loose, could be pulled out of the wall, and was accessible to children who are still placing objects in their mouths. Requirement: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths. Correction: To correct the metal gate loop being loose, a new wall board that is stronger was placed on the wall to more securely hold gate loop and it is no longer loose. Staff assigned to specific children (3280.113(a)(1))

Upon arrival at the facility on 5/26/26 at approximately 12:30 PM, Staff #1 was observed in the Room 3 (office) alone without any children. Once inside the facility, 1 older toddler was observed napping in Room 1 (next to the office). Staff #1 stated that they were responsible for the child in Room 1. Staff #1 stated that they believed they were compliant with supervision because Staff #1 was able to see the child in Room 1 from the office. At approximately 12:43 PM, Staff #1 walked into Room 2, leaving the child in Room 1 alone without a staff person present. Requirement: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Correction: The supervision concern was immediately addressed with staff. The staff member [#1] counseled regarding proper supervision procedures and reminded that children must remain under direct supervision at all times. Clean and good repair (3280.102(a))

In the middle room space, a ladybug bean bag was observed to contain ripped fabric, and not in good repair. Requirement: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. The most recent emergency drill on file at the facility is dated 8/23/24, more than 12 months prior to the date of inspection. Requirement: Emergency drills shall be conducted annually. Windex Glass Cleaner, rubbing alcohol, Awesome Cleaner, cleaner with bleach, and other toxics were observed in an unlocked closet and accessible to children in the hallway to the bathroom. Requirement: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. In the front entry area, broken plaster was observed on the top and bottom of the half wall where the gate attaches. The half wall in between the front room and middle room contained broken plaster where the gate attaches. The half wall in between the middle room and the back area contained broken plaster where the gate attaches. Requirement: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility. Correction: The plaster was replaced and painted over, also new screws were securely replaced, and the gate is replaced and in good condition. Inspection: Appeals - Unannounced Monitoring. All staff persons shall renew their certification in pediatric first-aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. In the middle room area, an electrical outlet under the shelves lacked an outlet cover. In the back-room area, an electrical outlet near the hanging headphones lacked an outlet cover. Requirement: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger. Building Surfaces (3280.76)

A sharp, exposed screw was observed on a gate in between the middle and back room, and could cause injury. Requirement: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards. Correction: The screw was taken out of the wall near the gate, and a repair man repaired the gate with no visible screws. Inspection: Appeals - Unannounced Monitoring. After communications with a representative of the school district, it was determined the diploma which was on file for staff #1 was fraudulent. Transcripts were provided by staff #1 the day following the investigation visit. After a review of the transcripts and communications with the same representative from [the school district], it has been determined the transcripts provided by staff #1 were not distributed by the district of the school listed on the transcript and are fraudulent. Requirement: The Department may deny, refuse to renew or revoke a certificate of compliance for fraud or deceit in obtaining or attempting to obtain a certificate of compliance. Correction: [Staff person #1] will never work in the facility with the children.
