Crib, port-a-crib, and playpen safety features

Licensing observed an infant sitting in a pack in play with a pacifier that had an attachment attached to the pacifier. Visit: Follow Up / Full. Plan to correct: Ms. Ray-Young immediately removed the attachment from the pacifier and will ensure that nothing is in the pack in play. Animals and household pets

At the time of licensing's monitoring visit, Buffalo (dog) did not have documented vaccination records maintained on the premises. Visit: Periodic / Full. Plan to correct: Provider contacted the vet and will get those emailed to me after a request has been sent. Provider will ensure that those are available and on site. The program does not have written permission from parent or guardian to transport during business hours. Complaint finding: Substantiated (complaint #18861). Plan to correct: I will continue to ensure that all permission slips to transport kids at our facility have been reviewed, signed by parents. Documentation is in each child's file. 340:110-3-87(e) — Passenger restraints and seating. Children remain properly secured in a child passenger restraint system, such as car or booster seat or individual seat belt, per Section 11-1112 of Title 47 of the Oklahoma Statutes (47 O.S. § 11-1112).

Transportation - The individual transporting children is not ensuring children are properly buckled into child passenger restraints or individual seat belts. Complaint finding: Substantiated (complaint #18861). Plan to correct: I have reviewed my transportation policy and will ensure all passengers are transported properly. This policy includes proper use of seat belts and child restraints. I will make sure the policy is followed each time a child is being transported from any staff at my facility. Licensing observed one assistant caregiver not having obtained at least the required number of professional development clock-hours to maintain a current Level 1 or higher OPDL certificate on site at the time of visit.
