Primary caregiver

The primary caregiver does not have a current PDL certificate as theirs expired on 1/23/26. Visit: Permit / Full. Plan to correct: Primary caregiver will complete at least 20 hours of professional development and will submitted documentation to the OK Registry. Primary will send a copy of her renewed PDL certificate once awarded to licensing. Moving forward, she will ensure her PDL certificate is current at all times at a level 4 or higher. General requirements

The provider did not have child information for one child that was in care today. Visit: Permit / Full. Plan to correct: Provider will get required forms and documents for the child and will keep them on site. One caregiver

When licensing arrived there was one adult present in the home with 10 children - three children under the age of two. Visit: Permit / Full. Plan to correct: The primary caregiver called parents/guardians for multiple children getting the home down to the correct ratio. Moving forward, provider will ensure the home is in ratio at all times by either not accepting children into care when it will put her over ratio, or by calling parents to come pick up children when the home becomes out of ratio due to staffing concerns.
