Additional Non-Compliance Found During Investigation: The program does not have written permission from parent or guardian to transport during business hours. Complaint finding: Substantiated (complaint #18871). Plan to correct: I will make sure I have documentation stating we have permission to transport. Home Environment - The home is not clean and sanitary. Complaint finding: Substantiated (complaint #18181). Plan to correct: In Supreme's defense daycare is very clean, no odor, very sanitize. Yes, there was a mice but client didn't see it. I yelled and ran she ask what was wrong I told her I saw a mice. I had an exterminator come out once a month until I moved.
