Licensing observed there to be 9 children present with one caregiver. 5 of the children present were 3 years of age and younger. Visit: Periodic / Full. Plan to correct: Ms. Lee stated she will work with her assistant for lunch breaks to ensure if she is needed for ratios that she remains at the program and takes a break during nap time without leaving the premises. Program was observed to be out of ratio on 12/02/25 with 3 of the 8 children being younger than 3 yrs old. Visit: Other / Partial. Plan to correct: Ms. Lee stated she was not planning on having her son that day and he was dropped off early due to an emergency. Ms. Lee stated she will ensure ratios are being maintained at all times at the program. Ms. Lee stated if this situation happens again were she would be out of ratio with her son being dropped off she will contact a family member to pick him up and watch him for her. Additional Non-Compliance Found During Investigation: Equipment: Mini-trampolines or mini-rebounders did not have a padded handle. Complaint finding: Substantiated (complaint #19521). Plan to correct: All mini trampolines in rebounders, where immediately removed from the childcare environment as they did not comply with Dhs licensing standards. I have reviewed the licensing regulations regarding equipment, safety, and now have a clear understanding of expectations related to trampoline use in the future. Any purchases of trampolines, rebounders or similar equipment will be submitted to Licensing for review and approval before used to ensure for compliance with safety requirements. Additional Non-Compliance Found During Investigation: Equipment: mini-trampolines or mini-trampolines used only by children 3 years of age and older were not being supervised. Complaint finding: Substantiated (complaint #19521). Plan to correct: The trampoline area has been permanently removed from the play space. as the sole caregiver I have reviewed the supervision standards outlined by Licensing and fully understand the importance of maintaining direct supervision during all gross motor activities. moving forward when additional staff are hired, I will train and demonstrate the importance of close supervision.
