Regulation: The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

At the time of inspection, in room 104, an infant had been placed in a crib that had a wedge underneath the mattress. The center has documentation from the infant's physician that stated the child should be kept in an upright position for 30 minutes after eating. The documentation does not state that a wedge should be placed in the crib with the infant. 3-4-2025 - The wedge has been removed from the crib in room 104. Ensure all crib mattresses are firm and snugly fitting in room 105. One crib mattress, where an infant was sleeping, was observed to be not properly fitting as there were gaps between the mattress and the sides of the crib. Regulation: Each crib or playpen shall be equipped with: a firm, waterproof, snugly fitting mattress; a clean, snugly fitting sheet; top rails that are at least 19 inches above the mattress and slats that are not more than 2 3/8 inches apart.
