Evidence of violation: During the review of documentation for the 2025 license renewal, it was discovered that a CBCU fingerprint memo had been altered. The licensing specialist confirmed with the CBCU administrator that a staff member has not had a current CBCU memo since 2023. Method of correction: Provider without a current CBCU memo cannot work until completed fingerprints are returned and submitted to the licensing specialist. Parents will need to be called to pick up children or an emergency designee will need to be called in to stay in ratio. Fiscal sanction if not corrected: $25.00 per calendar.
