Evidence of violation: A CPS report was filed, by another agency, regarding an incident involving the provider s nephew and an enrolled child. The provider was informed of the incident the day it happened, and the nephew confirmed the allegations. The provider failed to notify CPS as required. Method of correction: The state requires all providers, as mandated reporters, to report any suspected abuse to CPS. The licensing specialist is requiring the provider to complete North Dakota Mandated Reporter training to review types of abuse, recognize signs and indicators, and understand the required reporting procedures. Evidence of violation: During a review of documents for the renewal visits, It was determined that the owners fingerprints expired on June 21, 2025. Method of correction: The owner may not be on site during childcare hours until the CBCU results are returned. An emergency designee may continue care of the children until the fingerprints have been returned. The provider has 20 days to have fingerprints completed and returned.
