    Evidence of violation: on 4/6/26 an Unannounced visit was completed. Per disussion with CBCU Unit, it is being found that employee does not have a current results memo on file. She submitted her paperwork back in July 2024 but was closed unprocessed due to forms being rejected. Method of correction: Develop a tracking sheet to track background check dates, and all training dates to ensure documents don't expire. Time allowed for correction: 20 Day.     Evidence of violation: On 4/6/2026, I completed an Unannounced visit that is required once a ratio waiver is requested. When I arrived, there were 2 staff members present. I was told that Shyia stepped away for an emergency. Upon reviewing records/ratios, it was found that the two staff members were over ratio with a ratio of 2.66. During my visit, Shyia did return which put them back into ratio. Method of correction: Program needs to stay within ratio guidelines at all times. Develop a plan on how to handle medical emergencies or plan of how ratio guidelines will be handled if a staff member needs to step away. Time allowed for correction: Immediate.     Evidence of violation: During Unannounced visit that was conducted on 3/13/2025, it was found that both the supervisor and staff member have expired CPR/First Aid. They both expired 2/28/2025. Method of correction: Please enroll in an approved CPR/First Aid class that covers the topic areas of Pediatric/Infant. Upon completion, provide licensing specialist a copy of course. Supervisor will need a policy in writing of how they are going to monitor training courses to avoid expirations and non compliance of required trainings. Licensing specialist will be provided with a copy of the policy. Fiscal sanction if not corrected: $15.00 per day. Time allowed for correction: 20 Day.
