Correction order — Rule(s) 75-03-10-12-1(f) - Completion of one hour of department-approved safe sleep prior to the staff member providing care to infants;

Evidence of violation: Licensing specialist completed an unannounced visit on 4/7/2025. During the visit, it was determined that staff member has expired safe sleep training. Method of correction: Staff member must complete Safe Sleep training. Please keep a written record of staff trainings and when they are due. Time allowed for correction: 20 Day. Evidence of violation: During my renewal visit on 9/11/2024, licensing specialist found a 2 year old male child out on the playground unattended when i was leaving the facility. Licensing specialist estimates the child was left alone outside for approximately 10-15 minutes. Method of correction: Please provide licensing specialist with policies and procedures for outdoor play and how all children will be accounted for at all times.
