   It was observed that hazards were present in the indoor space. It was damp in the cabinet under the sink in the Oak Room and there was an odor and mold present on the washcloths, several safety latches on cabinets were not working properly, cords were accessible to children, and corners of rugs were curled posing a potential tripping hazard. Corrective action: We have cleaned under the sink and moved the rag that was present. We added a damp rid packet to keep the moisture under control. We have purchased three cord channels that will cover the air conditioner cords from the ac units to the plug. We purchased rug tape and will see to it that the rug corners will not curl to avoid a tripping hazard.. Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Indoor Space.    There were no records that evacuation drills were conducted at the program for May and June. Corrective action: We have added the two missing evacuation dates and talked with the Fire Chief in depth about evacuations and fire safety at the daycare. He states that we are doing a great job.. Regulation: The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits.    The program did not have a parent's signature on an Individual Health Care Plan for one child, and it did not have doctors consent for Benadryl for another child with an allergy. Corrective action: This missing signature was added and we have sent the Benadryl home until we have a more accurate Individualized Health Care Plan that includes first using Benadryl, then the Epi-Pen for allergies.
