   7.07(13)(d), 7.07(01), 7.07(02), 7.07(10) — Physical Facility - Indoors

   There were hazards in three classrooms including one shelf in each room was not installed and tippable and two cabinets were not locked and toxic substances were accessible to children. In the community room, the wood radiator cover was damaged in the bottom corner and it can easily open and cause an injury to children. Corrective action: Two pictures depict the locked cabinet in the Rainforest Classroom and the locking mechanism Another picture shows bleach and bleach wipes moved to the top of the wall cabinet, well out of reach of toddlers in the Meadow Room The shelving and kitchen set in the Forest and Rain Forest Rooms have been secured with "L" brackets. (see pictures) The issue in the community room has been addressed with duct tape covering the bottom of the wooden radiator cover and a child lock keeping it from being opened and accessed by children. (see picture). Regulation: All play equipment, fences and structures must be free of entrapment hazards., The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. Exits and evacuation routes must be kept clear of obstructions., Building Inspection. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). The certificate must indicate the following:, Indoor Space. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning.    7.07(07)(d), 7.07(07)(e), 7.07(10), 7.07(13)(d) — Physical Facility - Outdoors

   There were hazards in the preschool playground, including rust on the edges of the steps and platform on the climber, a pole at the top of the fence is not connected, and nails are protruding on the corners of the wood barrier surrounding an area. Corrective action: Any protruding nails were pulled out or hammered down and into the board. (See picture) The gap in the pole at the top of the chain link fence was remediated by placing a longer piece of PVC piping over it and a fabric netting was secured from the top of the fence pole to the bottom of the fence to cover the top fence pole as well as to provide privacy. (see picture) Rust on climber: Please refer to pictures of duct tape covering the area currently and the products we plan to use on the next dry day to remove any loose rust, seal the metal and spray with paint. We are waiting for an assessment from a local metal fabricating company in June for more permanent and aesthetically pleasing options. Regulation: The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate., If the outdoor play space is located on a roof, it must be protected by a barrier at least seven feet high, which cannot be climbed by children., Indoor Space.
