HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Training has expired and zero hours have been recorded for XXXXX. Training period's will be expiring for XXXXX in less than a month and both are approximately 5 hours short of meeting the 16 hours requirement. Please make sure that all staff have the appropriate hours of training, and certificates shall be submitted to PATCH for recording. Correction: All staff meets the H&S training.

HAR §17-892.2-3 — Background checks according with chapter 17-801 (criminal history, sex offender registry, child abuse/neglect, adult abuse perpetrator, employment history check, and other information)

Background checks for XXXXX has expired. Please turn in a completed DHS 948 Authorization for Background Check and to Release Information form. We will need the dates of XXXXX last employment with Seagull School Kailua. If XXXXX has been out of child care for 180 days or more, she will need to be fingerprinted again. HAR §17-892.1-3 — Inspection of facility, activities, records, staff

Child Care Licensing (CCL) Workers, Ryan Petersen and Tracy Oshita, conducted a scheduled site inspection visit on this day. Kama'aina Kids Enchanted Lake Preschool is moving to this new location, located at 410 Oneawa Street, Kailua, Hawaii 96734, formerly KCAA Atherton Preschool. This program will now be called, Kama'aina Kids Kailua Preschool. We met with Site Director, XXXXX and Executive Administrative Assistant, XXXXX to inspect the facility. We will need to return to conduct an observation of children in care. Correction: Worker conducted observation of children in care. Program meets licensing requirements.
