Correction: Worker received a copy of a DHS 974A Notification of Change Form signed by Education Program Manager XXXXX dated 11-26-24, stating that staff member XXXXX is no longer employed at this facility as of 11-26-24. As such, this deficiency has been satisfied. Copy of the form is on file.
