Please have new staff member XXXXX complete the Health & Safety Training initial assessment test and submit the completed form to PATCH. Correction: Email exchange between the worker and HCAP Program Manager XXXXX on 12-16-24 and 12-17-24: --XXXXX stated: " Initial Health & Safety Training - Tracy Kobashigawa : This was submitted to PATCH back in October". --Worker replied that a mistake has been made: "the Health & Safety Training deficiency was supposed to be for XXXXX, not XXXXX". --XXXXX replied that XXXXX never worked at this site, and she didn't know who that is. As such, XXXXX was removed from this site's staff listing, and this deficiency has been satisfied.
