Director to repaint the play structure and playhouse where there is peeling paint. Correction: The play structure and playhouse has been repainted.

HAR §17-892.2-63 — Furniture, equipment and toys shall be sturdily constructed with minimal hazards

Director to repair rust on the gate securing the electrical panel or install a barrier to keep children from the gate. Correction: The gate has been repainted.

HAR §17-892.2-63 — Sharp objects, household kitchen items, and tools kept out of reach or supervised when being used

Director to keep adult scissors out of reach of children. Correction: Director removed the scissors and place it out of reach of children (top shelf).

HAR §17-892.2-41 — Health record on each child

Director to obtain completed health records for nine children as discussed. Correction: Records are now complete.

HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Director to submit H&S certificates for two staff to PATCH to satisfy training requirements. Correction: Two staff members enrolled for the HIAEYC Conference on May 2, 2026 to earn their training certificates.

Director to obtain registration form for one child as discussed. Correction: Records are now complete.

HAR §17-892.2-63 — Potentially hazardous garbage and trash cans shall be properly covered

Director to cover trash can in the bathrooms. Correction: Director covered the trash cans.

Obtain completed health records from two children. Correction: Children's records are now complete.

HAR §17-892.1-33 — Pets, animals and fowl shall be maintained in a safe and sanitary way

Director to obtain written consent form from families to allow children to have contact with the pet turtle. Correction: Worker received copy of written consent form and on file

HAR §17-892.1-33 — No exposure to lead based paint

Director to repaint tricycles (rusting). Correction: On April 29, 2025, Director confirmed tricycles with rust have been removed from the playground and the preschool will purchase new bikes.

HAR §17-892.1-17 — All caregivers meet qualifications

Director to include staff to the HIEC registry. Imua Inclusion Preschool has the following qualified staff that have been cleared to work with children: 1 - Director/Teacher 2 - Teachers 1 - Asst. Teacher 1- Aide 1 - Substitute Teacher Correction: Worker verified staff is in HIEC Registry and qualification level inputted.
