HAR §17-896-18 — Health consultation provisions

Director to submit a written letter of agreement between the school and DOH public health nurse. Correction: Copy of a written agreement from DOH received and on file.

HAR §17-896-21 — One caregiver with current certificates during operating hours

Director to submit copy of a current Pediatric First Aid CPR card for staff members. There shall always be one caregiver with a current pediatric certificate when children are present. Correction: Verification of a Pediatric First Aid CPR received and on file (02/2025 valid two years).

HAR §17-896-31 — Potentially hazardous garbage and trash cans shall be properly covered

Director to cover the trash can in the classroom as discussed Correction: Classroom now has a covered trash can.

HAR §17-896-29 — Written evidence of fire warning device inspected initially and annually by fire inspector, report dated: <date>

Contact the MFD for an annual fire inspection of the facility and submit a copy of the report. Fire extinguishers were last serviced in July 2025. Correction: Per Assistant Director, fire inspection of the facility scheduled for October 2025.

HAR §17-896-31 — Child care area clean and comfortable

The Worker inspected the child care areas and found the indoor area clean and organized. All the classrooms and the cafeteria have air condition, so the entire indoor facility is kept at a comfortable temperature. The Worker also inspected the outdoor play areas which had enough shade for outdoor activities that was safe to play at. Outlets accessible to children need to be covered in all classrooms. Correction: Director covered all outlets.

Fire extinguishers last serviced in July 2024. The facility to contact MFD for an annual fire inspection and submit a completed satisfactory report. Correction: Fire inspection completed on 10/1/24 (report on file).
