HAR §17-892.2-3 — Annual Background Clearance Checks

The following employee's annual background clearances are expired: XXXXX Please have them complete and submit a DHS 948 AUTHORIZATION FOR BACKGROUND CHECK AND TO RELEASE FINDINGS form.

HAR §17-892.2-3 — Background checks according with chapter 17-801 (criminal history, sex offender registry, child abuse/neglect, adult abuse perpetrator, employment history check, and other information)

Please have XXXXX submit a completed DHS 948 Authorization for Background Check and to Release Information form immediately . Her annual clearance has expired. Correction: Worker verified on the HANA System that all staff hold current (not expired) annual background clearance checks at this time.

HAR §17-892.2-11 — Grievances, which includes department's information for parents to file complaints

The Parent Grievance Procedure is listed in the Head Start Family Handbook & Resource Guide page 26. However, the policy doesn't mention the contact information for the Child Care Licensing Office Unit 1 808-587-5266. Please submit a copy of a revised Head Start Family Handbook & Resource Guide which adds the contact info in the policy. Correction: Worker received pictures of satisfactory Grievance Policy posted on the classroom bulletin board

HAR §17-892.2-41 — Health record on each child

Please submit a letter from your health consultant or one of your staff attesting to the fact that they have reviewed all the children's health records and they have met the Head Start preschool requirements. Correction: Worker received a copy of an HCAP dated letter dated 1-29-26 from XXXXX stating that she has reviewed the HCAP health policies regarding the required immunization, TB Clearance and physical examinations and the medical status of all enrolled HCAP children in care have been reviewed to ensure that health & safety of the children are maintained. Copy of letter on file.

HAR §17-892.2-43 — One caregiver with current pediatric first aid certificate and child CPR during operating hours

Please submit copies of all your staff members First Aid/ Child CPR certificates. Correction: Worker received a copies of American Red Cross (ARC) Certificate of Completions for Adult and Pediatric First Aid/CPR/AED for the following staff: Teacher XXXXX Teacher XXXXX

HAR §17-892.2-61 — Evacuation

Please submit a copy of your site specific Natural Disaster Plan for KMCBH Head Start. The updated policy shall include sections that satisfy the new Hawaii Administrative Rules (HAR's) revised requirements NOTE: A copy of a Sample Disaster Plan Template that satisfies the new amended rules, is being sent with the Provisional License. Correction: 3-19-26: Worker received the following which satisfy the rule section ( §17-892.2-61 ) as listed above. 1. HCAP KMBCH Head Start Natural Disaster and Emergency Plans. School Year: 2025-26 2. HCAP Head Start & Early Head Start Emergency Procedures Guide Rev 2024 Copies have been placed on file along with a worksheet verifying this.

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

Please submit a current (not expired) Fire Inspection Report for the facility. Correction: Worker Received a copy of a Satisfactory Fire Inspection Report from CNRF-N611449, Fire Inspection Inspector. Initial Inspection was performed on 2-19-25 by Inspector. Report states that all deficiencies have been satisfied. NOTE: According to HCAP's Allison Yokoyama, email received on 9-5-24, the inspection report mistakenly refers to KMCBH as "YMCA" in Building 579, where KMCBH is located. See copy of Inspection Report and Ms. Yokoyama's email on file.

HAR §17-892.2-67 — Sanitation inspection dated: <date>

Please submit a current (not expired) Food Establishment Permit for the facility. Correction: Worker received a copy of Food Establishment Permit #030568, expires 9-24-26. Copy on file.
