HAR §17-892.1-3 — Child abuse/neglect check

HAR §17-892.1-17 — All caregivers meet qualifications

Please submit all on-going trainings to PATCH for the following employees: XXXXX. As a reminder all staff need 16 hours of approved training annually and substitutes need 10 hours of approved training annually. Correction: Worker verified that all employees have current health and safety training as of 10/23/25.

If they do not complete this requirement by 2/28/2026, we may exclude these staff members and they will not be able to count towards the staff-child ratio until they do, which may affect your license to operate a Group Child Care (GCC) program. HAR §17-892.1-6 — Others

HAR §17-892.1-29 — Physical exam clearance

Please submit the DHS 984 medical exam form and a TB clearance form for the following employees: XXXXX. Correction: Worker received copies of staff's physical and TB exams.

HAR §17-892.1-39 — Public beaches, swimming activities and wading pools

According to the Director, the program has an end of the school year beach day at 'Baby Makapu' beach. If the children attends, the parent is required to attend. Worker informed Director that they will need a certified lifeguard present at all times even if parents are required to attend with their child since it is a program function. Please provide the lifeguard's certificate that you will use during the field trip PRIOR to the event. Correction: Worker received a copy of lifeguard's XXXXX certificate that expires 7/16/2027.
