An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the previous inspection, the Licensing Specialist observed in the terrific two classroom the door frame to the exit door is missing the trim and there is a gap between the door frame and the wall. The drywall is also peeling. The baseboard on the back wall the trim is peeling off the wall and some of the paint is peeling off by the trim. In the bathroom the trim to the left of the door at the bottom is broken. In the Busy Bees classroom the changing table the top layer of the changing table is peeling and exposing the particle board. In the hallway between the one year olds and two year olds room the ceiling had a brown spot around the light. In the boys bathroom behind the toilet the tile is missing and exposing the inside of the wall and the door frame there is a gap between the door frame and the tiles exposing the wood. In the Disney 3 classroom the wall by the cots the paint is peeling. In the Bluey classroom the wall in circle time the paint is peeling. the seats in the 2013 vehicle are ripped and exposing the foam and the middle seat in the very last row is not attached to the flooring of the vehicle and can be moved. (The provider has requested for us to remove the 2013 vehicle at this time). During the reinspection, Licensing Specialist observed the facility repaired everything expect in the boys bathroom. The trim is still missing on the left side of the door frame and it is exposing the wood behind it. There is also a hole in the wall where the door stopper connects with the wall. This is being marked non-compliant. The director was given until May 8, 2026, to come into compliance with this standard. The facilitys outdoor play area contained litter, nails, glass or other hazards that posed a low potential for harm to children. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: 17-01: During the previous inspection, the Licensing Specialist observed on the pathway to the playground right outside the building there is bricks scattered with caution tape connected to green poles sticking out of the ground. The shed that is across from the preschool playground the bottom of the shed is deteriorating at the bottom of the shed and there were stack of hoses and mop buckets sitting by the buildings. On all the playgrounds the play equipment had a black and white substance on it. On the school age playground the metal wires that connects the fencing to the poles were coming off and have sharp pieces throughout the playground. There are bricks stacked on the back of the basketball hoop and the wheel to the basketball hoop is off. There is metal roof panels coming under the fence by the grey house. On the preschool playground the screws are sticking up out of the black boards, The steps to the jungle gym the covering is peeling. There is a hole by the fence by the bike area that was measuring at 3 inches in depth and 5 1/2 inches in width. On the 1 year old playground there were some torn foam equipment that was exposing the foam. During the reinspection, Licensing Specialist observed the facility repaired all items from previous inspection. During the reinspection, Licensing Specialist observed on the preschool playground that the black metal ties on the fence were not secured, sticking out and were sharp. One fence cap was not connected to the pole . This is being marked non-compliant. The director was given until May 8, 2026, to come into compliance with this standard. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: 32-7: During the inspection, Licensing Specialist observed that the school age outdoor play area did not the minimum 6 inch depth requirement around the permanent/stationary playground structures in that there were some sections within the 6 feet perimeter around the climber that had 0 inches of mulch. During the inspection, Licensing Specialist provided technical assistance, All permanent or stationary playground equipment must have a minimum of six (6) inches of resilient, impact-resistant ground cover such as sand, mulch, or artificial grass/turf or other protective surface such as rubber padding under the equipment and within the fall zone that provides resilience, and is maintained to reduce the incidence of injuries to children in the event of falls. Equipment used for climbing must not be placed over, or immediately next to, hard surfaces such as asphalt, concrete, dirt, grass, or flooring covered by carpet or gym mats not intended for use as surfacing for climbing equipment. All pieces of playground equipment should be placed over and surrounded by a shock-absorbing surface. TRAINING Compliance Comments During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 19 employees. During the time of the inspection, the Licensing Specialist monitored 19 of the 19 employee records. Counselor reviewed 10 hours of annual in-service, completed, for all child care personnel for the 2024/2025 fiscal year. Child Care personnel, returning to the industry and who were in compliance with training requirements when they left the industry, did not complete new mandated training within 90 days of employment. CCF Handbook, Section 4.3, A Training [SR] Comments: During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 21 employees. During the time of the inspection, the Licensing Specialist monitored 21 of the 21 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. 33-13: During the inspection, Licensing Specialist observed that there were 2 employees that did not complete the School Readiness Pre- Service training before they were left alone with the children. During the inspection, the staff employees were removed from the classroom. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, School Readiness Program Health and Safety Standards Handbook 3.1 Child care personnel must successfully complete the preservice training coursework described in this section, as applicable. All child care personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. This timeframe does not start over if personnel change employment to another School Readiness provider within this initial 90 days. Personnel who have not completed all preservice training requirements are not allowed any unsupervised contact with or care of children in a School Readiness program.
