The facilitys storage of harmful items including cleaning supplies, flammable products, and poisonous, toxic and hazardous materials allowed access by children in care. CCF Handbook, Section 3.2, B (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: 13-2: During the inspection, Licensing Specialist observed that the facility was storing chemicals in the storage unit outside, however the lock was not working allowing children to potentially have access to the chemicals. During the inspection, the chemicals were moved to a higher shelf. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.2, B All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times. 14. The ground cover or other protective surface under the [ the swings did not meet the 6 inch depth requirement, in that the poles of the swings are held in place by cement and the cement was visible through the mulch. ] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: 32-7: During the inspection, Licensing Specialist observed that the depth of mulch at the pole of the swings did not meet the 6 inch depth requirement, in that the poles of the swings are held in place by cement and the cement was visible through the mulch. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.12, D D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. 2. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Unitary surfaces must be tested to and comply with ASTM F1292; documentation of test data must be retained at the facility and available for licensing to review. 3. If the play area was approved by the Department prior to January 1, 2020 and does not meet the 6-foot fall/use zone requirement, then the facility must submit a written notification, including photographs and layout of the play area, to the Department prior to or on April 1, 2020, notifying its intention to continue to operate using the prior approved play area. However, if the permanent or stationary playground equipment is moved or replaced then the standard would apply, and 6 feet of use zone must be provided. 41. Immunization Records CCF Handbook, Section 7.1 (41-01)

Child(ren) did not have a Florida Certification of Immunization (DH Form 680) or a Religious Exemption from Immunization (DH Form 681), on file within 30 days of enrollment. CCF Handbook, Section 7.1, B Record Keeping [SR] Comments: During the time of the inspection, the Director advised the Licensing Specialist that there are currently 45 children enrolled, 10 of which had been newly enrolled since the last inspection. During the time of the inspection, the Licensing Specialist monitored 10 of the newly enrolled childrens records. During the time of inspection, Licensing Specialist observed 4 students that were missing immunization records. The Director was given until October 25th, 2024 to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1 B Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations.
