   The log for children transported did not include the following required information: [ signature of driver verifying all children left the vehicle]. FDCH/LFCCH Handbook, Section 2.4.2 Large Family [SR] Comments: During the inspection, the licensing specialist observed that some the transportation logs were missing the signature of the driver verifying all the children left the vehicle. The licensing specialist took pictures. This requirement was discussed the operator bring the issue into compliance. The licensing specialist provided technical assistance from the handbook Section 2.4.2 B. Upon arrival at the destination, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and 3. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted.    Indoor play areas were not inspected daily for basic health and safety and/or any problems were not corrected before the play area is used by children and/or daily inspection documentation was not maintained for 12 months. FDCH/LFCCH Handbook, Section 7, A Health Requirements [SR] Comments: During the inspection the licensing specialist observed the daily basic health and safety inspections were not documented for the months of February and March 2026. The licensing specialist took pictures. The operator advised the inspections were completed each day but did not them documented on the log.    The Florida Certificate of immunization was not acceptable in that: [ the form was not current (expired)]. FDCH/LFCCH Handbook, Section 8.1, A and B Children's Records [SR] Comments: At the time of the inspection, the specialist observed one childs file had an expired immunization record. The Licensing Specialist took pictures. The Licensing Specialist provided technical assistance from the handbook 8.1 8.1 A. The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C- 22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunization.
