CCF Handbook, Section 5.1, D (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: At time of the inspection, the Licensing Specialist observed one staff records did not have documentation of five years of employment history in the file. The Licensing Specialist took pictures. The Licensing Specialist provided technical assistance from the handbook 5.1, D The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information.     An unscreened individual was left alone with children in care. s.435.06(2)(a), F.S. and CCF Handbook, Section 5.2, C General Requirements [SR] Comments: During the inspection, the Licensing Specialist (LS) learned five children in the age one and two classroom were being supervised by a new staff member who did not have cleared background screening. The staff member was waiting for the results of fingerprinting. The director resolved the issue by promptly sending the staff member home and taking over supervision the children in that classroom. The (LS) explained to the director that staff must meet all the requirements in statue s. 435.06(2)(a) prior to any unsupervised contact with children. The staff member must have eligible results for fingerprints not still in process. Additionally explained that if the staff member had a provisional letter from background screening, they would still need to be supervised in the classroom until final clearance. Provided technical assistance from the handbook Section 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B. CF Form 1649A, Child Care Attestation of Good Moral Character, which is incorporated by reference in 65C-22.001(7)(a), F.A.C., must be completed for all child care personnel at the time of initial screening or upon change in employers. CF Form 1649A may be obtained from the Departments website at www.myflfamilies.com/childcare. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. D. CCF Handbook, Section 7.4.1,C Record Keeping [SR] Comments: At the time of the inspection, the Licensing Specialist (LS)observed one staff member did not have documentation of Level 2 Clearinghouse screening in the file. The staff member fingerprints were in process. The provider stated they thought the staff was approved provisionally. The (LS) explained the provider would need to have the provisional email in the record to document the department granted a provisional hire status. Prior to the completion of this report the provider emailed the licensing specialist documentation showing the staff member was eligible as of 12/18/2024. The licensing specialist reviewed the results on the background screening site that showed the staff member now eligible as of 12/18/2024.
