   The facility failed to notify the licensing authority of an incident or unusual occurrence that was threatening to the health, safety, or welfare of the children or child care personnel. CCF Handbook, Section 3.8.7 Comments: The facility must maintain a written plan for reporting any incident or unusual occurrences including injuries or illnesses requiring emergency medical treatment. The incident occurred on 2/19/2026 and it was reported to licensing on 3/12/2026.
