FDCH/LFCCH Handbook, Section 3.2,C Large Family [SR] Comments: Licensing Specialist determined through evidence & testimony that a substitute was working at the home day care on 12/17/2024, 12/18/2024, 2/6/2025, 2/24/2025 & 2/27/2025. The operator failed to report changes to the written substitute plan within five working days to Licensing Specialist. Licensing Specialist gave technical assistance from the Family Day Care Home and Large Family Child Care Home Handbook: 3.2 Substitute The operator must have a written plan to provide at least one substitute, 18 years of age or older, to be available on a temporary/emergency basis. C. Any changes to the plan must be reported to the licensing office within 5 working days. All hours worked by the substitute must be documented in writing and maintained for 12 months. Corrective Action: Provider made Licensing Specialist aware that a substitute was present for the days listed above during on-site visit on 3/20/2025. Provider stated that the substitute is no longer present a substitute with the large family day care home as of 2/27/2025. Provider also stated that the substitute was being utilized from an outside entity. Licensing Specialist observed written communication from the provider to the parent through the application of the incident that occurred on 2/6/2025. The application did not include a signature of the operator, signature of custodial parent or legal guardian & there was no way to determine that the parent received the note on the date of occurrence. Licensing Specialist gave technical assistance from the Family Day Care Home and Large Family Child Care Home Handbook: 7.20 Emergency Procedures and Notification The operator shall have a procedure for responding when an immediate emergency medical response is required. Emergency procedures must be posted and readily accessible. The operator shall develop contingency plans for emergencies or disaster situations when it may not be possible to follow standard emergency procedures. All child care personnel must be trained to manage in an emergency. D. All accidents, incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child, and observed health related signs and symptoms, which occur at a home, on field trips or during transportation, must be documented on the day they occur. Documentation shall include the name of the affected party, date and time of occurrence, description of occurrence, actions taken, and signature of operator and custodial parent or legal guardian. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence.    A Large Family Child Care Home may provide care for a maximum of: 1. 8 children from birth to 24 months of age. 2. 12 children with no more than 4 from birth to 24 months of age. There were [7] children observed in care and their ages were [1- Infant, 1- 2 year old, 2- 3 year old and 3- 4 year old]. 402.302 (11), F.S., FDCH/LFCCH Handbook, Section 2.2.2 [SR] Comments: Licensing Specialist observed, upon arrival the following ratio: 1 provider caring for a total of 7 children. (Ages: 1- Infant, 1- 2 year old, 2- 3 year old and 3- 4 year old) 1 Household member Within two minutes of the Licensing Specialist arrival to the large family day care home, Licensing Specialist observed a parent pick up 2 of the children from the large family day care home. Therefore, there were a total of 5 children left present during inspection. Licensing Specialist gave technical assistance from 402.302 (11), F.S., and the FDCH/LFCCH Handbook, Section 2.2.2: 402.302 (11) Large family child care home means an occupied residence in which child care is regularly provided for children from at least two unrelated families, which receives a payment, fee, or grant for any of the children receiving care, whether or not operated for profit, and which has at least two fulltime child care personnel on the premises during the hours of operation. One of the two fulltime child care personnel must be the owner or occupant of the residence. A large family child care home must first have operated as a licensed family day care home for 2 years, with an operator who has had a child development associate credential or its equivalent for 1 year, before seeking licensure as a large family child care home. Household children under 13 years of age, when on the premises of the large family child care home or on a field trip with children enrolled in child care, shall be included in the overall capacity of the licensed home. A large family child care home shall be allowed to provide care for one of the following groups of children, which shall include household children under 13 years of age: (a) A maximum of 8 children from birth to 24 months of age.
