03. Transportation FDCH/LFCCH Handbook, Section 2.4 [SR] (03-08)
The operator did not have evidence that a transportation log was maintained for all children transported. FDCH/LFCCH Handbook, Section 2.4.2 (Section 2.1 Health and Safety, Page 3) Comments: During the inspection, the Operator advised the Licensing Specialist that transportation services are o?ered to the children that are in care. The Operator advised that the children are transported in the 2021 Honda Pilot (019005). The transportation vehicle is insured through Progressive with an effective date of 5/13/25. The transportation vehicle was last inspected on 3/15/2025. During the inspection, Licensing Specialist observed the operator could not find her transportation log. The provider was given until October 5, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 2.4.2, Transportation Log- A log must be maintained for all children being transported in the vehicle or on foot away from and/or to the premises of the home. The log must be retained for a minimum of 12 months. The log must include each childs name, date, time of departure, time of arrival and the signature of the driver verifying all children were accounted for during the visual sweep. 07. Staff Training FDCH/LFCCH Handbook, Section 5 [SR] (07-05)
The home did not have at least one person providing care to children with a valid and current certification in pediatric CPR procedures and/or first aid training. FDCH/LFCCH Handbook, Section 5.1.1, C (Section 2.1 Health and Safety, Page 3) Comments: REMINDER: 10 hours of annual in-service training must be completed by renewal, 4/21/25. During the inspection, Licensing Specialist observed the owner CPR training expired 2/28/25. The provider was given until May 9, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 5.1.1, C Operators and Substitutes. C. Have certificate(s) of course completion for pediatric cardiopulmonary resuscitation (CPR) procedures and first aid training, which must be current and valid at all times. Certificates of course completion are valid based on the time frames established by each first aid and CPR training program, not to exceed three years. CPR courses must include an on-site instructor-based skills assessment that shall be documented by the certified CPR instructor. 13. Indoor Play Areas FDCH/LFCCH Handbook, Section 7.5 [SR] (13-03)
A health and safety deficiency having a low potential for harm to the children in care was observed. FDCH/LFCCH Handbook, Section 7.5, B (Section 2.1 Health and Safety, Page 3) Comments: During the inspection, Licensing specialist observed in the back room where the children play items stacked on top of each other(there were chairs stacked on top of each other, baby bouncer stacked on top pillows that was stacked on top of bags and other item which in turn stacked on top of a table. There was a stroller stacked on top of a chair and on top of the stroller there. The owner brought this standard back in compliance at the time of inspection by removing the items. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 7.5, B Indoor Play Areas B.
