Operator failed to maintain a current Employee/Contractor Roster for substitutes, employees or household members in the Clearinghouse. FDCH/LFCCH Handbook, Section 4.1, L Comments: During the inspection, the Licensing Specialist observed that the Family Day Care Homes Clearinghouse Employee/Contractor Roster was not current.The provider was given until March 13, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 4.1, L Initial Screening L. The employer/owner/operator must add substitutes, employees and household members to their Employee/Contractor Roster within five days of when the individual has received a child care eligible result. Employer/owner/operator must add an end date within five days of termination for individuals on the Employee/Contractor Roster in the Clearinghouse when employment terminates, or a household member is no longer residing in the home. The pet was not immunized though vaccinations are available for this type of animal. FDCH/LFCCH Handbook, Section 7.1 Comments: During the inspection, the Operator advised that there is one dog and one cat on the premises. During the inspection, the Operator provided the Licensing Specialist documentation of the dogs vaccinations that are valid until 01/27/2027. During the inspection, the Licensing Specialist observed the cats vaccination was expired on 11/27/25. The owner was given until March 13, 2026, to come into compliance with this standard. The provider was given extra time so she could get an appointment to update the cats vaccines. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section7.1 Animal Vaccinations Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the childs enrollment form. Outdoor play areas in the home were not free from litter, nails, glass, and other hazards. The provider has the pool drained and has a fence around the pool. The director was given until March 13, 2026, to come into compliance with this standard. During the inspection, the Operator had an operational corded phone. During the inspection, the licensing specialist observed that the fire extinguisher did not have a current certificate. The last date the fire extinguisher was tagged was on 9/2024. The provider was given until February 23, 2026, to come into compliance with this standard. In the event of an emergency, non-prescription medication that is not brought in by the custodial parent or legal guardian can be dispensed only if the operator has written authorization from the custodial parent or legal guardian to do so. Any medication dispensed under these conditions must be documented in the childs file and the custodial parent or legal guardian must be notified on the day of occurrence. The Operator has brought this standard back into compliance in that they removed the ripped mat and does not have children that need diapering currently enrolled.
