The provider presented vaccination records for the pets that were expired. Kitty examination date and vaccinations were completed on: 2/20/23. Expire 2/20/24. First pet: Kitty Kitty Species: Feline Age: 2 yrs. old and 3 months Breed: Domestic Longhair Coat Color: Gray Marbled Rabies tag number: 016080 Rabies Serial Number: 594846 Rabies brand name: RabVac 3 Max had an examination completed on 2/20/23. Vaccinations were given but expire on 2/20/24. Vaccinations given: Rabies, DHPP, Lepto, Bordetella, Heartworm, and Fecal. Second pet: Max Species: Canine Sex: Male Neutered Age: 3 yrs and 5 months Breed: Pug Coat color: Fawn Rabies tag number: 016080 Action Plan: Provider will set on her calendar a reminder of upcoming vaccination dates. Provider stated she was unaware it had to be done every year or has an expiration date. 7.1 Animal Vaccinations Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the child's enrollment form.
