An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Comments: During the inspection the licensing specialist observed there were areas in need of repair. The licensing specialist observed floor transition strip in the kitchen needed repair. Two areas of bathroom walls need repair where soap dispensers had been removed. The licensing specialist took pictures. The Licensing Specialist provided technical assistance from the handbook 3.1 General Health and Safety Requirements A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. Garbage containing food waste was not disposed of in a leak proof, nonabsorbent container with a tight fitting lid. CCF Handbook, Section 3.9.1, A.8. Food and Nutrition [SR] Comments: During the inspection, the licensing specialist observed the garbage container in the kitchen did not have a tight-fitting lid. The Licensing Specialist took pictures. The licensing specialist provided technical assistance from the handbook Section 3.9.1.A. 8.8.8. Leak-proof, non- absorbent containers covered with a tight-fitting lid for all food waste stored inside the facility. The container must be emptied, cleaned, and sanitized or disinfected daily. Compliance Comments During the inspection, the Operator advised that the facility provides meals and snacks to the children that are in care. The provider is a food program participant. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, the Licensing Specialist the operator did not have any documentation of Indoor play areas and equipment inspection before use and/or daily inspection documentation for the past 12 months. The director stated the log was discarded accidentally. The Licensing Specialist reviewed this requirement with the operator during the inspection, thus is marked back into compliance this date. The Licensing Specialist took pictures. The Director called the employer and completed the employment history request at the time of the inspection bringing the facility into compliance. The Licensing Specialist provided technical assistance from the handbook 5.1, D The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. Section 7.2, 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program.
