CCF Handbook Section 4.2.1 Comments: LS observed that the individual listed on the supplemental page that started in the industry on 11/14/2022 has not completed the Introduction Courses. The LS provided technical assistance (TA) to the director explaining that training must be completed within 12 months from the day employment begins in the child care industry. Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. TA was provided by the licensing specialist to the director explaining that blankets need to be stored in a sanitary manner and not touching. A safety deficiency having a moderate potential for harm to the children in care was observed. CCF Handbook, Section 3.1 Physical Environment [SR] Comments: November 13, 2025, the GOC 1 observed a video from November 4, that showed a childcare personnel member misusing a Boppy pillow to carry and move a babies around in the infant classroom, this should only be used to prop and position infants in care. Technical assistant was provided to the director to ensure that the manufacturer's instructions are reviewed prior to use. The video shows a teacher in the infant classroom, placing a baby on a Boppy pillow and picking the baby up with the Boppy pillow all in one to move the baby from one area of the classroom to another. The owner, operator, employee or substitute, while caring for children, committed an act or omission that meets the definition of child abuse or neglect provided in Chapter 39, Florida Statutes or Chapter 827, Florida Statutes. CCF Handbook, Section 8.2, A Enforcement [SR] Comments: November 13, 2025, the GOC1 reviewed a video showing the infant classroom on November 4, 2025. The teacher moved an infant from a feeding chair to a boppy pillow on the ground. The infant became fussy, so the teacher went back over to the infant, picked up the infant and the boppy pillow all in one and started to walk back over to where the other infants were. As she was holding the infant and the pillow, she crossed over the baby gate entry and dropped the infant from the boppy pillow onto the ground. This incident resulted in bruising to the infant's face. Technical assistance was provided to the teacher and the director to ensure that a Boppy pillow is not used to move babies from one place to another in the classroom and only used per the manufacturer instructions. Owner/Director/Staff Responsible Comments The provider stated that all the boppy pillows have been thrown out. The preschool has replaced them with two that are not able to be used to transport infants. Supervision of children in the [infant] group was inadequate in that [the teacher left an infant on the floor in the front of the classroom with a bottle while she attended to another infant in the classroom.]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: November 13, 2025, the GOC1 reviewed a video from November 4, 2025, that showed the teacher in the infant classroom place an infant on the front of the classroom floor with a bottle while she attended to another infant moving that infant around the classroom without direct supervision on the initial infant. Technical assistance was provided to the director to ensure that direct supervision is provided at all times for the infants in care. The video shows the teacher in the infant classroom, pick up a baby and place the baby on the floor in the front of the classroom by himself with a bottle. The teacher then moves to another baby and places that baby in a Boppy pillow to move the baby to the back of the classroom with her. A fire hazard, [in the air vents in rooms and bathroom], was observed in the facility. CCF Handbook, Section 3.1, B Physical Environment [SR] Comments: It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. This includes grease build-up in ovens, stoves and food equipment. Many A/C vents in all classrooms and bathroom have built up lint. This is a fire hazard. Please keep them clean.
